摘　　要：目的分析他汀类药物对华法林抗凝强度的影响,为临床合理用药提供参考依据。方法统计河北省人民医院2019年1月~8月所有使用华法林的门诊处方,抽取联合用药的处方进行整理,分析他汀类药物对华法林抗凝强度的影响。结果华法林合用他汀类药物处方共189张,联合使用氟伐他汀、辛伐他汀及瑞舒伐他汀前后INR差异均具有统计学意义(P <0.05),瑞舒伐他汀使INR增幅> 1的患者比例相似于氟伐他汀及辛伐他汀,但其使INR增幅> 0.5的患者比例较低。联合使用阿托伐他汀前后INR差异无统计学意义(P> 0.05)。结论氟伐他汀及辛伐他汀均可显著增加华法林的抗凝强度,瑞舒伐他汀对华法林抗凝强度的影响存在较大个体差异,阿托伐他汀不影响华法林的抗凝强度。Objective To analyze the effect of statins on the anticoagulant strength of warfarin in the outpatient prescription of a hospital,and to provide a reference for the rational use of drugs in the clinic. Methods All the outpatient prescriptions of warfarin in a hospital from January to August 2019 were counted. The prescriptions of combined medicines were collected and sorted out. The effect of statins on the anticoagulant strength of warfarin was statistically analyzed. Results A total of 189 prescriptions of warfarin combined with statins were selected. The INR before and after the combined use of fluvastatin, simvastatin and rosuvastatin were statistically different(P < 0.05). The proportion of patients whose INR increase greater than 1 were similar between rosuvastatin,fluvastatin and simvastatin, however, the proportion of patients whose INR increase greater than 0.5 caused by rosuvastatin was relatively low. There was no statistical difference in INR before and after the combined use of atorvastatin. Conclusion Fluvastatin and simvastatin can significantly increase the anticoagulant strength of warfarin. The influence of rosuvastatin on the anticoagulant strength of warfarin has large individual differences. Atorvastatin does not affect the anticoagulant strength of warfarin.
