2型糖尿病患者尿酸排泄分数和血清胆红素相关性分析

摘　　要：目的探讨2型糖尿病患者尿酸排泄分数与血清胆红素的相关性。方法选择2018年9月至2019年9月在我院就诊的50岁以上2型糖尿病患者585例,测定其血清胆红素、糖化血红蛋白(HbA1c)、血肌酐、血尿酸、24 h尿尿酸、尿肌酐等,计算尿酸排泄分数(FEUA)。根据FEUA值分为三组进行比较,采用方差分析、偏相关分析及多因素逐步回归分析等进行统计分析。结果方差分析显示,三组患者血尿酸、血清总胆红素(TBIL)、间接胆红素(IBIL)及直接胆红素(DBIL)比较,差异有统计学意义(F=5.822、43.069、44.363、22.335,均P<0.01)。偏相关分析显示,校正糖尿病病程、空腹血糖、HbA1c、年龄、体质量指数(BMI)及性别后,FEUA分别与TBIL、IBIL、DBIL呈负相关(r=-0.307、-0.304、-0.247,均P<0.01)。多因素逐步回归分析显示,TBIL、血尿酸、HbA1c及糖尿病病程是FEUA的影响因素(t=-8.377、-4.302、-3.201、2.607,均P<0.01)。结论2型糖尿病患者FEUA与TBIL、IBIL、DBIL呈负相关,而且TBIL是FEUA影响因素之一。Objective To explore the correlation between fraction excretion of uric acid and serum bilirubin in patients with type 2 diabetes.Methods A total of 585 patients with type 2 diabetes over 50 years old who were treated in our hospital from September 2018 to September 2019 were selected.The serum bilirubin,glycated hemoglobin(HbA1c),blood creatinine,blood uric acid,24 h uric acid,and urinary creatinine were detected.The fraction excretion of uric acid(FEUA)was calculated.According to the FEUA value,the patients were divided into three groups.Statistical analysis was performed using analysis of variance,partial correlation analysis,and multi-factor regression analysis.Results ANOVA showed that there were statistically significant differences in serum uric acid,total serum bilirubin(TBIL),indirect bilirubin(IBIL)and direct bilirubin(DBIL)between the three groups(F=5.822,43.069,44.363,22.335,all P<0.01).Partial correlation analysis showed that after adjusting for diabetes course,fasting blood glucose,HbA1c,age,body mass index(BMI)and gender,FEUA was negatively correlated with TBIL,IBIL,and DBIL(r=-0.307,-0.304,-0.247,all P<0.01).Multivariate stepwise regression analysis showed that TBIL,blood uric acid,HbA1c and diabetes course were the influencing factors of FEUA(t=-8.377,-4.302,-3.201,2.607,all P<0.01).Conclusion FEUA is negatively correlated with TBIL,IBIL,and DBIL in patients with type 2 diabetes,and TBIL is one of the influencing factors of FEUA.
