宫腔镜冷刀治疗子宫黏膜下肌瘤的临床效果及安全性

摘　　要：目的探讨宫腔镜冷刀治疗子宫黏膜下肌瘤的临床应用效果。方法选择2019年6月至2020年2月在我院行宫腔镜冷刀治疗的子宫黏膜下肌瘤患者30例为观察组,另选择2018年6月至2019年2月在我院行宫腔镜电切术治疗的子宫黏膜下肌瘤患者30例为对照组。比较两组手术时间、术中出血量、膨宫液用量、膨宫液吸收量、围术期及术后不良反应发生情况。结果观察组术中出血量、膨宫液用量、膨宫液吸收量均显著少于对照组,差异有统计学意义(P<0.05)。观察组0型肌瘤患者手术时间、术中出血量、膨宫液吸收量少于对照组,差异有统计学意义(P<0.05);观察组Ⅰ型与Ⅱ型肌瘤患者术中出血量、膨宫液用量、膨宫液吸收量少于对照组,差异有统计学意义(P<0.05)。观察组腹痛发生例数显著少于对照组,阴道排液时间显著短于对照组,差异有统计学意义(P<0.05)。结论宫腔镜冷刀作为一种新技术治疗子宫黏膜下肌瘤,具有操作简便、创伤小、术后恢复快等优点。Objective To explore the clinical application effect of cold knife under hysteroscopy in the treatment of uterine submucosal fibroids.Methods A total of 30 cases of uterine submucosal fibroids who underwent cold knife under hysteroscopy in our hospital from June 2019 to February 2020 were selected as the observation group,and 30 patients with uterine submucosal fibroids treated by electrosurgical resection under hysteroscopy from June 2018 to February 2019 in our hospital were selected as the control group.The operation time,the amount of intraoperative blood loss,the amount of uterine distention fluid,the uterine fluid absorption amount,and the adverse reactions in perioperative period and after the operation were compared between the two groups.Results The operation time,the amount of uterine distention fluid,and the uterine fluid absorption amount in the observation group were significantly less than those of the control group(P<0.05).The operation time,intraoperative blood loss,and uterine fluid absorption amount in patients with type 0 fibroids in the observation group were significantly less than those in the control group(P<0.05).In the observation group of typeⅠandⅡfibroids,the amount of intraoperative blood loss,the amount of uterine distention fluid and the absorption of uterine fluid were significantly less than those of the control group(P<0.05).The cases of abdominal pain in the observation group was significantly less than that in the control group,and the vaginal discharge time was significantly shorter than that in the control group(P<0.05).Conclusion Cold knife under hysteroscopy is a new technique for treating uterine submucosal fibroids.It has the advantages of simple operation,less trauma,and quick recovery after surgery.
