摘　　要：目的比较复苏后囊胚腔不同扩张程度对冻融周期移植单囊胚移植妊娠结局的影响。方法回顾性分析于2018年1月-2019年12月在本中心行冻融单囊胚移植1139例患者资料,根据复苏后囊胚腔扩张程度分为扩张组、不完全扩张组和未扩张组,采用二分类Logistic回归分析的方法探讨囊胚腔不同扩张程度对妊娠结局的影响。结果囊胚腔完全扩张组的移植周期生化妊娠率、临床妊娠率、早期流产率和继续妊娠率均高于不完全扩张组和未扩张组,差异有统计学意义(P<0.05)。二分类Logistic回归分析结果表明,复苏后的囊胚腔不同扩张级别对生化妊娠率有显著影响,扩张级别越低结果越显著[OR=6.0495%CI(3.05-11.95),P<0.001]。结论冻融单囊胚移植周期中,复苏后的囊胚腔扩张级别可有效预测妊娠结局。Objective:To compare the effect of blastocyst expansion degree on pregnancy outcome of single blastocyst transplantation in freeze-thaw cycles.Methods:A retrospective analysis was performed on the data of 1139 patients who received freeze-thawed single blastocyst transplantation in our center from January 2018 to December 2019.According to the degree of blastocyst expansion after cryopreservation and thawing,the patients were divided into the expanded group,the incomplete expanded group and the non-expanded group.Regression analysis was used to investigate the effect of different degrees of blastocyst expansion on pregnancy outcome.Results:The biochemical pregnancy rate,clinical pregnancy rate,early abortion rate and ongoing pregnancy rate of the complete expansion group were higher than that of the incomplete expansion group and the non-expansion group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that different expansion levels of the resuscitated blastocyst cavity had significant effects on the biochemical pregnancy rate.The lower the blastocyst expansion level,the more significant the result[OR=6.0495%CI(3.05-11.95),P<0.001].Conclusion:In the freeze-thaw single blastocyst transplantation cycles,the degree of resuspated blastocyst expansion grade can effectively predict the pregnancy outcome.
