多囊卵巢综合征不孕症采用未成熟卵母细胞体外成熟与常规体外受精-胚胎移植技术治疗的疗效比较

摘　　要：目的比较体外成熟(IVM)和IVF对多囊卵巢综合征(PCOS)患者治疗效果。方法回顾性分析IVM(235)和IVF(420)对多囊卵巢综合征不孕患者实验室和临床结局。结果IVM组周期移植率为95.4%,IVF组周期移植率为81.9%,差异显著(P<0.01)。两组获得卵母细胞数无显著差异(P>0.05);IVM组成熟卵母细胞数、可用胚胎数和胚胎种植率均显著低于IVF组(P<0.01)。两组之间的受精率和卵裂率无显著差异。IVM获得的临床妊娠率和流产率高于IVF组,活产率低于IVF组,但无显著差异(P>0.05)。结论IVM可能是PCOS不孕症患者IVF的有效替代治疗方法,它能消除OHSS的风险,降低药物成本,同时保持较高的临床妊娠率。Objective:To compare the laboratory and clinical outcomes of in vitro maturation(IVM)and IVF on polycystic ovary syndrome(PCOS).Methods:The laboratory and clinical outcomes of IVM(235)and IVF(420)in infertile women with PCOS were analyzed retrospectively.Results:the rate of ET cycle was 95.4%in IVM group and 81.9%in IVF group(P<0.01).There was no significant difference in the number of oocytes obtained between the two groups(P>0.05);the number of mature oocytes,available embryos and embryo implantation rate in IVM group were significantly lower than those in IVF group(P<0.01).There was no significant difference in fertilization rate and cleavage rate between the two groups.The clinical pregnancy rate and abortion rate of IVM group were higher than those of IVF group,and the live birth rate was lower than that of IVF group,but there was no significant difference(P>0.05).Conclusion:IVM may be an effective alternative to IVF in patients with PCOS patients with the reduced OHSS risk drug cost and a acceptable clinical pregnancy rate.
