普济消毒饮联合泼尼松治疗亚急性甲状腺炎疗效及对血清TRAb及TPOAb水平的影响

摘　　要：目的:观察普济消毒饮联合泼尼松治疗亚急性甲状腺炎的疗效及对血清促甲状腺激素受体抗体(TRAb)、甲状腺过氧化物酶抗体(TPOAb)水平的影响。方法:选取亚急性甲状腺炎患者75例作为研究对象,依据随机数字表法分为对照组及研究组,对照组37例均服用泼尼松片,研究组38例在对照组的基础上增服普济消毒饮。评价2组临床疗效,检测血清TRAb、TPOAb水平,并比较甲状腺功能水平及复发情况。结果:研究组的总有效率为94.74%,高于对照组的75.68%(P<0.05)。与同组治疗前比较,治疗后2组血清TRAb、TPOAb及游离三碘甲状腺原氨酸(FT_(3))、游离四碘甲状腺原氨酸(FT_(4))降低(P<0.05),且研究组低于对照组(P<0.05)。与同组治疗前比较,治疗后2组促甲状腺激素(TSH)水平升高(P<0.05),且研究组高于对照组(P<0.05)。停药6个月后随访,研究组复发率为5.00%,低于对照组的40.00%(P<0.05)。结论:普济消毒饮联合泼尼松治疗亚急性甲状腺炎,能提升整体疗效,利于改善甲状腺功能,降低复发率。Objective:To observe the curative effect of Puji Xiaodu yin combined with prednisone on patients with subacute thyroiditis and its effect on levels of thyrotropin receptor antibody(TRAb) and thyroid peroxidase antibody(TPOAb) in serum. Methods:A total of 75 cases of patients with subacute thyroiditis were selected as the study objects. All patients were divided into the control group and the study group according to the random number table method,37 and 38 cases in each group respectively. The control group was given the oral administration of prednisone tablets,and the study group was additionally given the oral administration of Puji Xiaodu yin based on the treatment of the control group. The clinical effect was evaluated in the two groups. The levels of TRAb and TPOAb in serum were detected. The level of the thyroid function and the recurrence were compared. Results:The total effective rate was 94.74% in the study group,higher than that of 75.68% in the control group(P<0.05). When compared with those in the same group before treatment,the levels of TRAb,TPOAb,free triiodothyronine(FT_(3)),and free thyroxine(FT_(4)) in serum in the two groups after treatment were decreased(P<0.05);the levels in the study group were lower than those in the control group(P<0.05). When compared with those in the same group before treatment,the levels of thyroid stimulating hormone(TSH) in the two groups after treatment were increased(P<0.05);the level in the study group was higher than that in the control group(P<0.05). The follow-up was conducted after six-month drug withdrawal. During the follow-up,the recurrence rate was 5.00% in the study group,lower than that of 40.00% in the control group(P<0.05). Conclusion:In the treatment of subacute thyroiditis,the therapy of Puji Xiaodu yin combined with prednisone can improve general curative effect,reduce the recurrence rate,and conduce to improving the thyroid function.
