血栓通注射液联合利伐沙班预防膝关节置换术后下肢深静脉血栓临床研究  

Clinical Study on Xueshuantong Injection Combined with Rivaroxaban in Preventing Deep Venous Thrombosis After Knee Arthroplasty

摘　　要：目的:观察血栓通注射液联合利伐沙班预防膝关节置换术后下肢深静脉血栓的临床效果。方法:选择行膝关节置换术患者72例,随机分为对照组与观察组各36例。对照组给予利伐沙班治疗,观察组在对照组的基础上联合血栓通注射液治疗。比较2组治疗前后同型半胱氨酸(Hcy)、D-二聚体(DD)及超敏C-反应蛋白(hs-CRP)、红细胞压积、全血黏度、纤维蛋白原水平变化;观察2组治疗后1周、2周下肢深静脉血栓发生情况;记录2组治疗期间不良反应情况。结果:治疗后,2组红细胞压积、全血黏度、纤维蛋白原水平较治疗前明显降低(P<0.05),且观察组上述各项指标均低于对照组(P<0.05)。治疗后,2组DD、Hcy及hs-CRP水平均较治疗前明显降低(P<0.05),且观察组DD、Hcy、hs-CRP水平明显低于对照组(P<0.05)。治疗后1周、2周,观察组下肢深静脉血栓发生率分别为0、5.56%,对照组分别为11.11%、22.22%,2组比较,差异有统计学意义(P<0.05)。观察组不良反应发生率为5.56%,对照组为8.33%,2组比较,差异无统计学意义(P>0.05)。结论:血栓通注射液联合利伐沙班片应用于膝关节置换术后,可显著改善患者血液流变力学指标,有效预防下肢深静脉血栓的发生。Objective:To observe the clinical effect of Xueshuantong injection combined with rivaroxaban in preventing deep venous thrombosis after knee arthroplasty. Methods:A total of 72 cases of patient undergoing knee arthroplasty were divided into the control group and the observation group according to the random number table method,36 cases in each group. The control group was treated with rivaroxaban,and the observation group was additionally treated with Xueshuantong injection based on the treatment of the control group. Before and after treatment, the changes in levels of homocysteine(Hcy),D-dimer(DD),high-sensitivity C-reactive protein(hs-CRP),hematocrit,whole blood viscosity and fibrinogen in the two groups were compared. One and two weeks after treatment, the incidences of deep venous thrombosis in the two groups were observed. During treatment,the adverse reactions in the two groups were recorded. Results:After treatment,the levels of hematocrit,whole blood viscosity and fibrinogen in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the above indexes in the observation group were lower than those in the control group(P<0.05). After treatment,the levels of DD,Hcy and hs-CRP in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the above levels in the observation group were significantly lower than those in the control group(P<0.05). One and two weeks after treatment,the incidences of deep venous thrombosis were 0 and 5.56% respectively in the observation group,and 11.11% and 22.22% respectively in the control group,the difference being significant(P<0.05). The incidence of adverse reactions was 5.56% in the observation group and 8.33% in the control group, there being no significance in the difference(P>0.05). Conclusion:The application of Xueshuantong injection combined with rivaroxaban tablets after knee arthroplasty can significantly improve hemorheology indexes and effectively prevent the occurrence of deep venous
