摘 要:目的检测先天性心脏病(CHD)伴肺动脉高压(PH)患儿血清中缺氧诱导因子1α(HIF-1α)、微小RNA-210(miR-210)的变化及意义。方法选取2016年8月-2018年9月山西省儿童医院收治的125例CHD患儿作为研究对象,其中单纯CHD患儿28例为单纯CHD组,CHD伴PH患儿97例,根据PH严重程度分CHD伴轻度PH组患儿33例、CHD伴中度PH组患儿34例、CHD伴重度PH组患儿30例。比较4组患儿一般资料、右心导管部分检查指标及检测血清中HIF-1α、miR-210水平,分析HIF-1α与miR-210的相关性;分析影响CHD伴PH的影响因素。结果4组患儿性别、年龄、HR、SaO2、SBP、DBP比较差异无统计学意义(P>0.05)。与单纯CHD组相比,CHD伴(轻、中、重)度PH组SPAP、DPAP、MPAP、MRAP、SRVP、PVR及血清中HIF-1α、miR-210水平升高,差异有统计学意义(P<0.05)。与CHD伴轻度PH组相比,CHD伴(中、重)度PH组SPAP、DPAP、MPAP、SRVP、PVR及血清中HIF-1α、miR-210水平升高,差异有统计学意义(P<0.05)。与CHD伴中度PH组相比,CHD伴重度PH组SPAP、DPAP、MPAP、SRVP、PVR及血清中HIF-1α、miR-210水平升高,差异有统计学意义(P<0.05)。CHD患者血清中HIF-1α与miR-210呈明显正相关(P<0.05),MPAP、SRVP、HIF-1α、miR-210是影响CHD患者合并PH的危险因素(P<0.05)。结论CHD伴PH相较于单纯CHD患儿血清中HIF-1α、miR-210高表达,且随着PH的严重HIF-1α、miR-210水平越高,两者呈正相关;是影响CHD患者伴PH的危险因素,可能与CHD患者并发PH关系密切。Objective To detect the changes and significance of serum hypoxia-inducible factor-1α(HIF-1α)and microRNA-210(miR-210)in children with congenital heart disease(CHD)and pulmonary hypertension(PH).Methods From August 2016 to September 2018,125 children with CHD were selected from Children's Hospital of Shanxi and divided into simple CHD group(28 children)and CHD+PH group(97 children),the children in CHD+PH group were divided into CHD+mild PH group(33 children),CHD+moderate PH group(34 children),and CHD+severe PH group(30 children)according to the severity of PH.The general data,examination indexes of right cardiac catheters,the levels of serum HIF-1αand miR-210 in the four groups were compared.The correlation between HIF-1αand miR-210 was analyzed.The influencing factors of CHD and PH were analyzed.Results There was no statistically significant difference in gender,age,HR,SaO2,SBP,and DBP among the four groups(P>0.05).Compared with simple CHD group,the levels of SPAP,DPAP,MPAP,MRAP,SRVP,PVR,serum HIF-1αand miR-210 in CHD+PH group increased significantly(P<0.05).Compared with CHD+mild PH group,the levels of SPAP,DPAP,MPAP,SRVP,PVR,serum HIF-1αand miR-210 in CHD+moderate PH group and CHD+severe PH group increased significantly(P<0.05).Compared with CHD+moderate PH group,the levels of SPAP,DPAP,MPAP,SRVP,PVR,serum HIF-1αand miR-210 in CHD+severe PH group increased significantly(P<0.05).The level of serum HIF-1αwas positively correlated with serum miR-210 in CHD children(P<0.05).MPAP,SRVP,HIF-1α,and miR-210 were risk factors of PH in CHD children(P<0.05).Conclusion Compared with simple CHD children,the levels of serum HIF-1αand miR-210 in children with CHD and PH are higher,which increase with the aggravation of PH,HIF-1αis positively correlated with miR-210.HIF-1αand miR-210 are risk factors of children with CHD and PH,which may be closely related to CHD children complicated with PH.
