摘　　要：目的探讨彩色多普勒超声检测晚孕期胎儿颅内及颈动脉血液动力学指标对宫内窘迫的诊断和预测价值。方法选取2018年3月-2019年3月在我院就诊的胎儿窘迫孕妇患者57例,分为胎儿宫内窘迫组,选取同时期在我院体检的正常妊娠孕妇60例,分为正常组。采用彩色多普勒超声检查胎儿的颅内及颈动脉血液动力学指标的血流变化情况,分析对宫内诊断和预测价值。结果胎儿颅内血流动力学与正常组相比,胎儿宫内窘迫组PSV、EDV水平明显上升,PSV、PI、RI水平明显下降(P<0.05);胎儿颈动脉血液动力学与正常组相比,胎儿宫内窘迫组PSV、PSV、PI、RI水平明显上升,EDV水平明显下降(P<0.05);超声对胎儿宫内窘迫的诊断灵敏度为94.74%(54/57),特异度为84.21%(16/19),正确率86.33%(101/117);ROC曲线分析显示,五项联合对血液动力学指标诊断价值较高(P<0.05)。结论彩色多普勒超声检查对胎儿宫内窘迫的诊断价值较高,且颅内及颈动脉血液指标对宫内窘迫的预测价值,可用于晚孕期宫内窘迫的诊断和预测。Objective:To investigate the value of color Doppler ultrasound in the diagnosis and prediction of fetal intracranial and carotid hemodynamics in late pregnancy.Methods:In this study,57 pregnant women with fetal distress were selected from March 2018 to March 2019 in our hospital,They were divided into fetal distress group,Sixty pregnant women with normal pregnancy who had physical examination in our hospital at the same time were selected,They were divided into normal group.Color Doppler ultrasound was used to examine the hemodynamic changes of intracranial and carotid arteries,Objective to analyze the value of intrauterine diagnosis and prediction.Results:Compared with the normal group,the levels of PSV and EDV in fetal distress group were significantly increased,and the levels of PSV,PI and RI were significantly decreased(P<0.05);Compared with the normal group,the levels of PSV,PSV,PI and RI in fetal distress group were significantly increased,while EDV levels were significantly decreased(P<0.05);The sensitivity,specificity and accuracy of ultrasonography were 94.74%(54 / 57),84.21%(16 / 19)and 86.33%(101 / 117),respectively;ROC curve analysis showed that the combination of five items had higher diagnostic value for hemodynamic indexes(P<0.05).Conclusion:The value of color Doppler ultrasound in the diagnosis of fetal distress is high,and the intracranial and arterial blood indexes can be used for the diagnosis and prediction of intrauterine distress in late pregnancy.
