摘  要:目的分析2018年广西壮族自治区防城港市流行性腮腺炎(流腮)疫情中K基因型腮腺炎病毒(Mumps virus,MuV)分离株的SH基因特征。方法从2018年防城港市小学流腮疫情中采集病例血清和口腔拭子,分别采用酶联免疫吸附试验和实时荧光逆转录-聚合酶链反应检测MuV IgM/IgG抗体和核酸,对阳性核酸样本的SH基因进行扩增和测序,分析其基因特征。结果在采集样本的流腮病例中,MuV IgM、IgG抗体阳性率分别为68.2%(15/22)、100%(22/22),MuV核酸阳性率为80.8%(21/26)。选取10株MuV进行SH基因核苷酸序列分析,经鉴定均为K基因型,核苷酸同源性为99.6%-100%;其与2016年中国广西和越南K基因型株、中国S79疫苗株的核苷酸同源性分别为99.0%-99.6%、84.4%-84.8%。MuV分离株的某些氨基酸位点发生了突变。结论2018年广西防城港市流腮疫情为K基因型MuV引起;与越南K基因型MuV亲缘关系密切。需加强中越边境地区含腮腺炎成分疫苗接种和流腮监测处置工作。Objective To describe characteristics of the SH genes of K genotype mumps viruses(MuV) isoloated from mumps outbreaks in Fangchenggang city of Guangxi Zhuang Autonomous Region in 2018.Methods We collected serum and oral swab specimens from mumps cases in primary school outbreaks in Fangchenggang.We tested for MuV IgM and IgG using enzyme-linked immunosorbent assay and for MuV ribonucleic acid(RNA) using real-time reverse transcription-polymerase chain reaction(RT-PCR).We amplified and sequenced the SH genes of RNA-positive specimens to determine genetic characteristics.Results Sero-positivity rates of MuV IgM and IgG among cases from whom specimens were collected were 68.2%(15/22) and 100%(22/22),respectively,with 80.8%(21/26) RT-PCR positivity for MuV RNA.Ten MuV strains were identified as genotype K by nucleotide sequencing analysis of SH genes.Within-strain nucleotide homology was 99.6%-100%;between-strain homology with the genotype K reference strains from Guangxi in 2016 and Vietnam was 99.0%-99.6%,and between-strain homology with the S79 vaccine strain was 84.4%-84.8%.Mutations were seen at some amino acid sites.Conclusion The mumps outbreaks in Fangchenggang in 2018 were caused by K genotype MuV,which had a close genetic relationship with the Vietnam K genotype MuV We should enhance mumps vaccination and surveillance in China-Vietnam border areas.
