辽宁省免疫策略转换前后三种脊髓灰质炎疫苗疑似预防接种异常反应比较分析

摘　　要：目的比较辽宁省脊髓灰质炎(脊灰)疫苗免疫策略转换前后不同脊灰疫苗疑似预防接种异常反应(AEFI)发生率。方法通过中国AEFI监测系统收集辽宁省脊灰疫苗免疫策略转换前(2013年5月-2016年4月)三价脊灰减毒活疫苗(tOPV)和转换后(2016年5月-2019年12月)二价脊灰减毒活疫苗(bOPV)、脊灰灭活疫苗(IPV)AEFI数据,比较AEFI报告发生率。结果辽宁省免疫策略调整前tOPV和调整后bOPV和IPV的AEFI报告发生率(/10万)分别为6.03、11.11、27.80,其中一般反应分别为5.44、10.14、26.16,异常反应分别为0.28、0.62、1.29。在tOPV、bOPV、IPV异常反应中,过敏性皮疹报告发生率(/10万)分别为0.10、0.47、1.17,疫苗相关麻痹型脊灰报告发生率(/10万)分别为0.07、0.04、0.00。结论辽宁省脊灰疫苗免疫策略调整前后三种脊灰疫苗不良反应发生率在预期范围内,严重反应极为罕见。Objective To compare the incidence of adverse event following immunization(AEFI) with different poliomyelitis vaccines before and after the polio immunization strategy shift in Liaoning province.Methods We obtained AEFI data for trivalent oral poliomyelitis attenuated live vaccine(tOPV) before the shift(May 2013 to April2016),and bivalent oral poliomyelitis attenuated live vaccine(bOPV) and inactivated poliovirus vaccine(IPV) after the shift(May 2016 to December 2019) that were reported to the National AEFI Surveillance System.We compared AEFI incidences of the three vaccines.Results The AEFI incidences per 100 000 doses for pre-shift tOPV,post-shift bOPV,and post-shift IPV were 6.03,11.11,and 27.80,respectively,with 5.44,10.14,and 26.16 for common vaccine reactions,and 0.28,0.62 and 1.29 for rare vaccine reactions.Among rare vaccine reactions for tOPV,bOPV,and IPV,incidences per 100 000 doses for allergic rash were 0.10,0.47,and 1.17,and were 0.07,0.04,and 0.00 for vaccine-associated paralytic poliomyelitis.Conclusion The incidences of AEFI for the three polio vaccines were acceptable before and after the polio immunization shift in Liaoning.Severe reactions were extremely rare.
