摘　　要：目的比较不同出生体重儿童免疫规划(EPI)疫苗预防接种不良反应报告发生率。方法通过灌云县妇幼保健系统和全国疑似预防接种异常反应监测信息管理系统,收集灌云县2015-2016年新生儿出生体重和9种EPI疫苗不良反应数据,比较不同出生体重儿不良反应发生率。结果在灌云县2015-2016年出生低体重儿、正常体重儿、巨大儿中,9种EPI疫苗总不良反应发生率(/万剂)分别为2.31(2例)、5.48(179例)、7.04(36例)(x2=3.62,P=0.17),一般反应分别为2.31、5.27、6.45(x2=2.69,P=0.26),异常反应分别为0.00、0.21、0.59(Fisher确切概率法,P=0.32),卡介苗(BCG)不良反应分别为0.00、0.00、7.05(Fisher确切概率法,P=0.02);其他EPI疫苗不良反应发生率在三组儿童之间均无显著性差异。结论低出生体重儿接种EPI疫苗未增加不良反应发生风险,巨大儿BCG不良反应发生风险可能高于正常体重儿。Objective To compare incidences of adverse reactions following immunization with Expanded Program on Immunization(EPI) vaccines among children with different birth weights.Methods We obtained data on birth weights and adverse reactions to any of nine EPI vaccines among children of Guanyun county during 2015-2016 that were reported to the Guanyun Maternal and Child Health Care System and the National Information System for Surveillance of Adverse Events Following Immunization.We compared incidences of adverse reactions by birth weight group.Results Among low-,normal-,and high-birth-weight children born in 2015-2016 in Guanyun,overall incidences per 10 000 doses of adverse reactions for 9 EPI vaccines were 2.31(2 cases),5.48(179 cases),and 7.04(36 cases),respectively(x2=3.62,P=0.17),with 2.31,5.27,and 6.45(x2=2.69,P=0.26) for common adverse reactions,and 0.00,0.21,and 0.59(Fisher’s exact test,P=0.32) for rare adverse reactions.The incidences of adverse reactions for BCG vaccine were 0.00,0.00,and 7.05(Fisher’ s exact test,P=0.02) among the three birth weight groups;incidences for the other EPI vaccines were not significantly different by birth weight group.Conclusion Low birth weight did not increase adverse reactions following vaccination with EPI vaccines among children,but the risk of BCG adverse reactions may be higher in high-birth-weight children than in children with normal birth weights.
