摘 要:目的探讨在基因水平上地佐辛对瑞芬太尼诱发痛觉过敏的大鼠脊髓Gs蛋白的影响。方法成年雄性SD大鼠,体重200~250 g,12~14周龄,32只,随机分成4组8例,空白对照组(C组)、手术组(S组)、瑞芬太尼组(R组)、地佐辛组(D组);C组大鼠尾静脉持续泵注盐酸瑞芬太尼同体积的静脉用0.9%Nacl,持续60 min;S组大鼠麻醉后制备切口痛模型,同时尾静脉泵注生理盐水60 min;R组大鼠麻醉后,静脉注射地佐辛同体积生理盐水0.1 ml,随后制备切口痛模型,同时泵注瑞芬太尼1.3μg/(kg·min)持续60 min;D组大鼠麻醉后静脉注射地佐辛1 mg/kg,随后制备切口痛模型后,同时持续静脉泵注瑞芬太尼1.3μg/(kg·min),持续60 min。检测4组大鼠给药前及给药后2 h的热痛刺激潜伏期(PWTL)及大鼠腰膨大处脊髓中Gs蛋白mRNA表达水平。结果 (1)行为学结果:给药前各组大鼠PWTL无差异(P>0.05)。给药后,S组和R组PWTL短于C组(P<0.05),D组PWTL与C组无差异(P>0.05);与S组相比,R组PWTL缩短,D组PWTL延长(P<0.05);与R组相比,D组PWTL延长。(2)Gs蛋白mRNA表达结果:与S组相比,R组Gs mRNA表达上调(P<0.05);与D组相比,R组Gs mRNA水平升高(P<0.05)。结论地佐辛可抑制瑞芬太尼诱发大鼠痛觉过敏,同时脊髓Gs蛋白表达降低。Objective To clarify the mechanism of dezocin alleviating remifentanil-induced hyperalgesia in rat.Methods Male SD rats were divided into four groups(n=8)by using a random number table.Control group(group C):continuous pumped normal saline via caudal vein 60 min;In group S,the model of incisional pain was established,and the equal volume of normal saline was infused for 60 min via the tail vein;Group R:the model of incisional pain was established,and remifentanil was infused for 60 min via the tail vein at a rate of 1.3μg/(kg·min)at the same time.in group D:the model of incisional pain was established,intravenous inject dezocine 1 mg/kg before infusing remifentanil.Before drugs administration and 2 hours after drugs administration,respectively paw withdrawal thermal latency(PWTL)was detected on the rats’right hindpaw in all groups.After behavior test,the expression of Gs mRNA in spinal cord was determined by RT-PCR.Results(1)The result of behavioral tests:There was no statistically difference on PWTL of each group before administration(P>0.05).After administration,Compared with group C,PWTL was significant shorten in group S and R(P<0.05);Compared with group S,PWTL was significant shorten in group R(P<0.05);Compared with group R,PWTL was lengthen in group D(P<0.05).(2)The result of Gs mRNA:Compared with group S,the expression of Gs mRNA was up-regulated in group R(P<0.05).Compared with group R,the expression of Gs mRNA was down-regulated in group D(P<0.05).ConclusionThe mechanism by which Dezocin could mitigate remifentanil-induced hyperalgesia may be related to inhibiting the expression of Gs in rats.
