摘　　要：目的探讨医用胶原蛋白海绵在Quadrant可扩张管通道技术治疗腰椎管狭窄症中的临床疗效。方法收集2018年5月至2019年5月经Quadrant可扩张管通道技术治疗腰椎管狭窄的患者78例,随机分为对照组和治疗组,对照组采用传统方式止血38例,治疗组采用医用胶原蛋白海绵合并传统方式止血40例。对2组患者临床病历资料进行统计分析,并记录和比较2组患者手术时间、术中出血量、术后下地时间、术后引流量,并发症及术后住院时间,术前1 d、术后3 d及1、3、6个月和1年疼痛视觉模拟评分(VAS)及Oswestry功能障碍指数(ODI)等手术资料,按照MacNab腰椎功能标准评价2组临床疗效。结果2组患者性别比、年龄、病程差异无统计学意义(P>0.05);手术时间,术中出血量,术后下地时间,术后引流量,术后3 d和1个月VAS评分,差异有统计学意义(P<0.05)。2组患者术后住院时间,术前1 d,术后3个月、6个月及术后1年VAS评分和ODI评分,差异无统计学意义(P>0.05)。结论医用胶原蛋白海绵在Quadrant可扩张管通道技术治疗腰椎管狭窄症中的应用可有效降低手术时间、减少术中出血和术后引流量,加快术后康复。Objective To investigate the clinical effects of medical collagen sponge in treatment of lumbar spinal stenosis(LSS)by Quadrant expandable tube channel technique.Methods A total of 78 patients with LSS who were treated by Quadrant expandable tube channel technique in our hospital from May 2018 to May 2019 were randomly divided into control group(n=38)and treatment group(n=40).The patients in control group were treated by traditional hemostasis,however,the patients in treatment group were treated by medical collagen sponge combined with traditional hemostasis.The clinical medical records of the two groups were statistically analyzed,and the duration of operation,intraoperative bleeding volume,postoperative off-bed time,postoperative drainage,complications,and postoperative hospital stay were recorded and compared between the two groups.The visual analogue scale(VAS)for pain and Oswestry dysfunction index(ODI)at 1d before operation and at 1,3,6,and 12 months after operation were recorded,and the clinical curative effects were evaluated according to the MacNab standard of lumbar spine function.Results There were no significant differences in patient's sex and age,and course of disease between the two groups(P>0.05).Significant difference was found in duration of operation,intraoperative blood loss,postoperative off-bed time,postoperative drainage,and VAS scores at 3 days and 1 month after operation(P<0.05).There was no significant difference in postoperative hospital stay as well as VAS score and ODI score at 1d before operation and at 3m,6m and 12m after operation between the two groups(P>0.05).Conclusion In the treatment of LSS by Quadrant expandable tube channel technique,the application of medical collagen sponge can effectively reduce the duration of operation,intraoperative bleeding and postoperative drainage,and accelerate postoperative recovery.
