经颅多普勒检测儿童病毒性脑炎脑血流变化及与患儿预后的相关性分析

摘　　要：目的:分析经颅多普勒(Transcranial Doppler,TCD)检测儿童病毒性脑炎脑血流变化及与患儿预后的相关性。方法:回顾性分析2017年1月-2019年1月郑州大学附属儿童医院收治的病毒性脑炎患儿100例临床资料,根据治疗15d后的格拉斯格量表(GOS)评定结果将患儿分为预后优良组(62例)和预后不良组(38例)。两组均行TCD检测,对比分析两组患儿的脑血流动力学情况,并分析其与预后不良的相关性。结果:预后不良组MCA的Vs、Vd,PCA与ACA的Vs、Vd及PI均高于预后优良组,差异有统计学意义(P<0.05);经Pearson分析,MCA的Vs、Vd与患儿预后呈正相关(r>0,P<0.05);PCA的Vs、Vd及PI与预后情况呈正相关(r>0,P<0.05);ACA的Vs、Vd及PI与预后效果呈正相关(r>0,P<0.05)。结论:TCD检测儿童病毒性脑炎脑血流动力学指标与患儿预后效果呈正相关,可通过其变化情况评估患儿短期预后状况。Objective:To analyze the changes of cerebral blood flow in children with viral encephalitis detected by transcranial Doppler(TCD)and its correlation with the prognosis of children.Methods:The clinical data of 100 children with viral encephalitis treated in the hospital from January 2017 to January 2019 were retrospectively analyzed.The children were classified as excellent prognosis according to the results of the Glasg Scale(GOS)after 15 days of treatment group(62cases)and poor prognosis group(38cases).Both groups were tested for TCD.The cerebral hemodynamics of the two groups of children were compared and analyzed,and their correlation with poor prognosis was analyzed.Results:Vs,Vd,PCA of MCA and Vs,Vd,and PI of ACA were higher in the poor prognosis group than in the good prognosis group,and the differences were statistically significant(P<0.05).According to Pearson analysis,Vs,Vd of MCA and the prognosis of the children had positive correlation(r>0,P<0.05);Vs,Vd and PI of PCA were positively correlated with prognosis effect(r>0,P<0.05).Vs,Vd and PI of ACA were positively correlated with prognostic effect(r>0,P<0.05).Conclusion:TCD detection of cerebral hemodynamic parameters in children with viral encephalitis is positively correlated with the prognosis of children.The short-term prognosis of children can be evaluated by its changes.
