机器人辅助下腹腔镜前列腺癌根治术后麻醉恢复质量的临床分析

摘　　要：目的评价机器人辅助下腹腔镜前列腺癌根治术后麻醉恢复质量的影响因素。方法搜集前列腺癌根治术患者60例,将患者分为两组:手术时间≤2 h组(S)、手术时间>2 h组(L),术后送麻醉复苏室(PACU)拔管后进行镇静程度评分。统计清醒拔管时间及出PACU时间,VAS疼痛评分和出现恶心呕吐的次数。分析影响麻醉恢复质量的因素与手术时间,年龄,体质量指数(BMI)等相关性。结果2组患者中S组的清醒拔管时间短于L组(P<0.05),出PACU时间也短于L组(P<0.01)。患者的清醒拔管时间与手术时间,出PACU时间与年龄皆有相关性(P<0.01),VAS与BMI无相关性(P>0.05)。结论前列腺癌患者可根据患者特征和外科医生的经验选择机器人辅助下微创手术治疗,术前通过预测影响麻醉恢复的因素如手术时间,年龄等,对于高风险患者提前制定相关预防措施,以促进该类患者快速康复,并减少术后并发症。Objective To evaluate the factors influencing the quality of anesthesia recovery after robot assisted laparoscopic radical prostatectomy.Methods Sixty patients having undergone radical prostatectomy were divided into two groups:L Group(operation time ≤2 h) and S group(operation time> 2 h).The degree of anesthesia in both groups was assessed after extubation in the postanesthesia care unit(PACU).The time of awake extubation,time of discharge out of PACU,scores by visual analogue scale(VAS) and frequency of nausea and vomiting were recorded and analyzed to look into the correlations of the influencing factors of anesthesia recovery quality with the operation duration,age,body mass index(BMI).Results The awake extubation time ofthe S groupwas shorter than that of the L group(P <0.05),and the time of discharge out of PACU in the S group was shorter than that of the L group(P <0.01).The awake extubation time was significantly correlated with the operation duration,time of discharge out of PACU and age(P <0.01),but insignificantly with VAS and BMI(P> 0.05).Conclusion The robot assisted minimally invasive surgery is optional for patients with prostate cancer upon the characteristics of patients and the experience of surgeons.Before operation,it is suggested to predict those influencing factors such as operation time and age and even make precautions measures for those patients with high risks so as to promote their rehabilitation and reduce the incidences of complications.
