摘  要:目的:以自发性高血压大鼠(SHR)为模型,观察不同负荷剂量高强度间歇训练(HIIT)对肾脏损伤的影响并探讨细胞外基质(ECM)代谢与肾小管上皮-间充质转分化(EMT)在其间的可能作用机制,为高血压肾病患者完善健康管理以及优化运动康复方案提供依据。方法:45只雄性SHR按照随机数字表分为安静组(SHR-Sed)、低负荷剂量HIIT组(SHR-loHIIT)和高负荷剂量HIIT组(SHR-hiHIIT),同时将10只Wistar-Kyoto大鼠作为正常血压对照组(WKY)。WKY和SHR-Sed组在鼠笼内安静饲养;SHR-loHIIT和SHR-hiHIIT组分别进行18周不同负荷量HIIT,两组运动强度保持一致,但SHR-loHIIT组总负荷剂量仅为SHR-hiHIIT组的1/4。实验后,利用无创血压仪检测尾动脉血压;测定24 h尿蛋白、血尿素氮(BUN)和血清肌酐(SCr)含量评价肾功能;过碘酸-希夫(PAS)和马森(Masson)染色进行肾脏病理组织学观察并获取肾小球和肾小管损伤评分以及纤维化指数;透射电子显微镜观察肾脏组织超微结构;免疫印迹测定ECM合成因子—转化生长因子β1(TGF-β1)、磷酸化Smad2/3(p-Smad2/3)、结缔组织生长因子(CTGF)以及ECM降解因子—基质金属蛋白酶-9(MMP-9)和组织金属蛋白酶抑制物-1(TIMP-1)蛋白表达量;免疫组织化学染色检测肾小管EMT标志物—α-平滑肌肌动蛋白(α-SMA)和E-钙黏蛋白(E-CA)的分布与表达量。结果:1)肾脏损伤与纤维化:与WKY组比较,SHR-Sed组血压升高,肾小球萎缩、基底膜增厚、间质胶原纤维大量沉积,足细胞数量减少、足突融合破坏,肾小球和肾小管损伤评分以及纤维化指数升高,肾功能下降(P<0.05);与SHR-Sed组比较,SHR-loHIIT组血压降低,肾小球和肾小管病变减轻,纤维化指数以及组织损伤评分下降,肾功能改善(P<0.05),而SHR-hiHIIT组血压水平、病理组织学变化、组织损伤评分以及纤维化指数进一步升高(P<0.05),但肾功能无显著性变化(P>0.05)。2)ECM代谢通路:与WKY组比较,SHR-Sed组TGF-Objective:Spontaneous hypertensive rats(SHR)were used as animal models to explore the effect of different load doses of high-intensity interval training(HIIT)on renal fibrosis and investigate the possible mechanism of extracellular matrix(ECM)metabolism and renal tubular epithelial-mesenchymal transition(EMT),so as to provide the optimal exercise protocol for the health management of patients with hypertensive nephropathy.Methods:45 male SHR were divided into sedentary(SHR-sed),low load-dose(SHR-loHIIT)or high load-dose(SHR-hiHIIT)groups according to a random number table,at the same time,ten Wistar Kyoto rats were used as normal blood pressure control group(WKY).Animals in WKY and SHR-Sed groups were bred quietly in cages,and those of SHR-loHIIT and SHR-hiHIIT groups performed 18-week different load dose of HIIT.Training intensity maintained consistent but the total load dose of SHR-loHIIT group was only 1/4 of SHR-hiHIIT group.After the experiment,parameters were measured including blood pressure of the tail artery by non-invasive blood pressure monitor;renal function by 24 h urine protein(UP),blood urea nitrogen(BUN)and serum creatinine(SCr)content;histopathological observation by periodic acid-Schiff(PAS)and Masson staining to obtain glomerular and tubular injury score and fibrosis index;ultrastructure of kidney tissue by transmission electron microscope;protein expression levels of ECM synthesis factors such as transforming growth factorβ1(TGF-β1),phosphorylated Smad2/3(p-Smad2/3)and connective tissue growth factor(CTGF),and ECM degradation factors such as matrix metalloproteinase-9(MMP-9)and tissue metalloproteinase inhibitor-1(TIMP-1)by Western blot;distribution and expression of smooth muscle actin(α-SMA)and E-cadherin(E-CA)by immunohistochemical staining.Results:1)Renal injury and fibrosis:Compared with WKY group,SHR-Sed group has increased blood pressure,glomerular atrophy,thickening of the basement membrane,large deposition of interstitial collagen fibers,reduced number of podocytes,foot processes f
