摘 要:目的:探讨尿铜水平与空腹血糖异常的关系,为糖尿病的预防提供理论依据。方法:该研究基于广西少数民族自然人群慢性病队列研究,纳入广西南宁市壮族居民3 888名,通过问卷调查和体格检查收集研究对象基线资料,同时采集调查对象的血样和尿样。通过生化分析仪对血葡萄糖进行检测,电感耦合等离子体质谱仪检测尿中铜含量。应用多因素logistic回归分析尿铜水平与空腹血糖异常的关系,限制性立方样条分析尿铜浓度与空腹血糖异常的关系。结果:调查对象尿铜浓度中位数为16.61μg/L,空腹血糖异常率为6.5%。年龄、体重指数(Ibm)以及腹部肥胖在空腹血糖正常和异常组间存在差异(P<0.05)。调整混杂因素后,尿铜浓度>P75时与空腹血糖异常的相关性是尿铜浓度≤P25的1.988倍(OR=1.988,95%CI:1.358~2.910,P<0.001)。尿铜浓度与空腹血糖异常呈线性关系(P=0.094),随着尿铜浓度的增高,与空腹血糖异常的关联强度呈上升趋势(P<0.001)。结论:研究人群尿铜水平较高,尿铜水平与空腹血糖异常呈正相关。Objective: To investigate the relationship between urinary copper level and abnormal fasting blood glucose, and provide evidence for the prevention of diabetes. Methods: The study was based on the cohort of chronic diseases of natural population of minority nationalities in Guangxi, and included 3,888 Zhuang residents in Nanning, Guangxi. The baseline data of the participants were collected by questionnaires and physical examination. The blood samples and urine samples of the subjects were collected at the same time. The blood glucose level was detected by biochemical analyzer. And the copper content in urine was detected by inductively coupled plasma mass spectrometer. Multivariate logistic regression was used to analyze the relationship between urinary copper level and abnormal fasting blood glucose. The restrictive cubic spline was used to analyze the relationship between urinary copper concentration and abnormal abnormal fasting blood glucose. Results: The median of urine copper concentration was 16.61 μg/L in the research populations. And their abnormal rate of fasting blood glucose was 6.5%. Age, body mass index and abdominal obesity were different in people with normal and abnormal fasting glucose(P<0.05). After adjusting for confounding factors, the correlation between urinary copper concentration>P75 and abnormal fasting blood glucose was 1.988 times higher than that of urinary copper concentration ≤ P25.(OR=1.988, 95% CI:1.358-2.910, P<0.001). Alinear relationship between urinary copper concentration and abnormal fasting blood glucose was observed(P=0.094), and the correlation intensity with abnormal fasting blood glucose increased with the increase of urine copper concentration(P<0.001). Conclusion: The urine copper level in this population is high and it is positively correlated with abnormal fasting blood glucose.
