不同低密度脂蛋白胆固醇水平冠心病患者视黄醇结合蛋白4与SYNTAX积分的相关性

摘　　要：目的通过测定血清视黄醇结合蛋白4(RBP4)和计算SYNTAX积分,探讨不同低密度脂蛋白胆固醇(LDL-C)水平冠心病患者血清RBP4与冠脉病变SYNTAX积分的相关性。方法选取100例住院行冠脉造影的冠心病患者,根据LDL-C水平达标与否分为LDL-C水平达标组(A组:LDL-C<1.8 mmol·L^(-1),50例)与非LDL-C水平达标组(B组:LDL-C≥1.8 mmol·L^(-1),50例)。收集患者一般资料,比较2组血清甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、LDL-C及RBP4水平。根据冠状动脉病变的不同程度和部位进行SYNTAX积分,分析2组RBP4水平与冠状动脉病变SYNTAX积分的相关性。结果A组RBP4水平与SYNTAX积分无关(r=0.032,P>0.05);B组RBP4水平与SYNTAX积分呈正相关(r=0.571,P<0.01);A+B组100例患者RBP4水平与SYNTAX积分呈正相关(r=0.358,P<0.05)。结论RBP4在LDL-C水平升高的基础上与冠心病冠脉病变SYNTAX积分显著正相关,对非LDL-C水平达标冠心病冠脉病变程度有较高的预测价值。Objective To explore the correlation between serum retinol-binding protein 4(RBP4)and SYNTAX score in coronary artery disease patients with different levels of low-density lipoprotein cholesterol(LDL-C).Methods One hundred hospitalized patients with coronary heart disease who underwent coronary angiography were divided into two groups:ideal LDL-C level group(group A,LDL-C<1.8 mmol·L^(-1),50 patients)and non-ideal LDL-C level group(group B,LDL-C≥1.8 mmol·L^(-1),50 patients).Serum triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),LDL-C and RBP4 levels were compared between the two groups.The SYNTAX score was calculated according to the degree and location of coronary artery disease,and the correlation between RBP4 level and SYNTAX score was analyzed.Results There was no significant correlation between RBP4 level and SYNTAX score in group A(r=0.032,P>0.05).However,RBP4 level was positively correlated with SYNTAX score in group B and group A+B(r=0.571、P<0.01,r=0.358、P<0.05).Conclusion RBP4 is positively correlated with SYNTAX score in coronary heart disease patients with elevated LDL-C levels.Therefore,RBP4 is of predictive value in evaluating the degree of coronary artery disease in patients with high LDL-C levels.
