摘  要:目的探讨不同速度泵注生理盐水对经瓣膜式PICC引导腔内心电图稳定性与准确性的影响。方法选取在贵州中医药大学附属医院2020年3-7月行三向瓣膜式PICC置管的肿瘤患者120例。采用随机数字表法分为A、B、C3组每组各40例,置管静脉均选择右侧贵要静脉,采用微量注射泵泵注不同速度的生理盐水。将A、B、C3组生理盐水泵注速度分别设为50 mL/h、60 mL/h、70 mL/h。分别观察并记录各组腔内心电图稳定性、置管准确性、生理盐水用量和操作耗时。结果A、B、C3组患者导管尖端到位准确率分别为75.00%、80.00%、95.00%,心电图稳定率分别为75.00%、85.00%、95.00%,生理盐水平均用量分别为(12.39±0.83)mL、(11.00±0.71)mL、(9.80±1.08)mL,平均操作耗时分别为(14.87±0.99)min、(11.00±0.71)min、(8.40±0.93)min。结论当生理盐水泵注速度为70 mL/h时可引导出更稳定、连续的腔内心电波形,导管尖端到位准确性较高。Objective To investigate the effect of pumping normal saline at different speeds on the stability and accuracy of electrocardiogram in valvular PICC guided cavity.Method A total of 120 tumor patients who received three-way valvular PICC catheterization in our hospital from March to July 2020 were selected.The patients were randomly divided into A,B and C groups.The right basilic vein was selected for catheterization,and normal saline at different speeds was injected with micro injection pump.The infusion speeds of the three groups of physiological salt water pumps were set as 50mL/h,60mL/h and 70mL/h respectively.The stability of electrocardiogram,the accuracy of catheterization,the dosage of normal saline and the operation time of each group were observed and recorded.Result In group A,B and C,the catheter tip placement accuracy was 75.00%,80.00%and 95.00%,the electrocardiogram stability rate was 75.00%,85.00%and 95.00%respectively,the average dosage of normal saline was 12.39±0.83mL,11.00±0.71mL and 9.80±1.08mL respectively.The average operation time was 14.87±0.99 min,11.00±0.71 min and 8.40±0.93 min,respectively.Conclusion When the infusion speed of physiological salt pump is 70mL/h,it can lead to more stable and continuous intracavitary electric waveform,and the catheter tip is in place with high accuracy.
