系统性红斑狼疮合并癫痫临床特征及治疗前后免疫球蛋白的表达

摘　　要：目的探讨系统性红斑狼疮合并癫痫患者的临床特征及治疗前后免疫球蛋白的表达水平.方法将160例系统性红斑狼疮患者按照是否合并癫痫分为合并组80例与未合并组80例,将合并组按照随机数字表法分为观察组与对照组,各40例.合并组给予卡马西平治疗,对照组在此基础上给予强的松治疗,观察组在对照组基础上给予环磷酰胺治疗,观察3个月.比较合并组及未合并组患者的临床特征,比较治疗前后观察组及对照组患者免疫球蛋白A、免疫球蛋白G及补体C3、C4水平,比较治疗前后观察组及对照组白细胞介素-6、白细胞介素-8及肿瘤坏死因子水平.结果合并组病程<1 a、白细胞计数上升、补体C4降低、尿红细胞上升、尿素氮上升、肌酐上升、血清白球比降低、丙氨酸氨基转移酶上升、门冬氨酸氨基转移酶上升患者均显著多于未合并组(P<0.05或0.01).治疗后观察组及对照组免疫球蛋白A、免疫球蛋白G水平显著低于治疗前(P<0.01),补体C3、C4水平显著高于治疗前(P<0.01),观察组较对照组变化更显著(P<0.01).治疗后观察组及对照组白细胞介素-6、白细胞介素-8、肿瘤坏死因子水平均显著低于治疗前(P<0.01),观察组较对照组下降更显著(P<0.01).结论系统性红斑狼疮合并癫痫患者具有明显的临床特征,采用强的松联合环磷酰胺治疗可使免疫球蛋白及炎症因子水平显著降低,补体水平显著升高.Objective To investigate the clinical characteristics of systemic lupus erythematosus complicated with epilepsy and expression of immunoglobulin before and after treatment.Methods A total of 160 patients with SLE were divided into the combined group and the uncombined group according to w hether they had epilepsy,80 cases each.The combined group was divided into observation group and control group according to the random number table method,40 cases each.The combined group was treated with carbamazepine,the control group was given prednisone treatment,and the observation group was given cyclophosphamide on the basis of the control group for 3 months.The clinical characteristics of the patients in the combined group and the uncombined group were compared.The levels of IgA,IgG and complement C3 and C4 in the observation group and the control group were compared before and after treatment.The levels of IL-6,IL-8 and TNF-αin the observation group and the control group were compared before and after treatment.Results The cases numbers of patients with disease course<1 year,increased white blood cell,decreased complement C4,increased urine ery throcyte,increased urea nitrogen,in-creased creatinine,decreased serum white ball ratio,increased alanine aminotransferase and increased aspartate aminotransferase in combined group were higher than those in non-combined group(P<0.05 or 0.01).The levels of IgA and IgG in the two groups were significantly lower than those before treatment(P<0.01),and the levels of complement C3 and C4 were significantly higher than those before treatment(P<0.01).The observation group was more signifi-cant than the control group(P<0.01).The levels of IL-6,IL-8 and TNF-αin the two groups were significantly lower than those before treatment(P<0.01),and the observation group decreased more sig-nificantly than the control group(P<0.01).Conclusions Patients with systemic lupus ery thematosus and epilepsy have obvious clinical features.Treatment with prednisone combined with cyclophosphamide can signif
