老年永久起搏器置入患者术后起搏器系统优化效果分析

摘　　要：目的探讨老年永久起搏器置入患者术后起搏器系统优化的效果.方法对200例老年永久起搏器置入患者进行为期1年的随访,随访期间进行起搏系统优化干预,比较干预前后左室射血分数、左室舒张末期容积、二尖瓣舒张早期血流峰值流速、二尖瓣舒张晚期血流峰值流速、6 min步行距离、纽约心脏病协会心功能分级,采用日常生活活动能力量表及生存质量量表评定患者的日常生活活动能力及生活质量,统计患者满意率.结果干预后患者左室射血分数及左室舒张末期容积水平均显著高于干预前(P<0.05或0.01),二尖瓣舒张早期血流峰值流速及二尖瓣舒张晚期血流峰值流速与干预前比较差异无统计学意义(P>0.05),6 min步行距离显著长于干预前(P<0.01),纽约心脏病协会心功能分级显著优于干预前(P<0.01).干预后患者日常生活活动能力量表及生存质量量表评分均显著高于干预前(P<0.01),患者总满意率为94.0%.结论起搏器系统优化能够改善老年永久起搏器置入患者的心功能、日常生活活动能力和生存质量,患者满意度较高.Objective To explore the effect of postoperative pacemaker system optimization in elderly patients with permanent pacemaker implantation.Methods The clinical data of 200 elderly patients with permanent pacemaker implantation were collected.All patients were given a 1-year follow-up.During follow-up,the patients were given pacemaker system optimization.Before and after intervention,LVEF,LVEDV,mitral valve diastolic blood flow peak flow velocity,mitral valve diastolic blood flow peak flow velocity,6 min walking distance,NYHA cardiac function grading of the two groups were compared.The ADL and QOL were used to assess the activities of daily living and quality of life of patients,and the patient satisfaction rate was calculated.Results After intervention,the levels of LVEF and LVEDV were significantly higher than those before intervention(P<0.05 or 0.01).There was no significant difference between A and E compared with before intervention(P〉0.05).The 6-minute walking distance was significantly longer than that before the intervention(P<0.01),and the NYHA cardiac function grading was significantly better than that before the intervention(P<0.01).The ADL and QOL scores of the patients were significantly higher than those before the intervention(P<0.01),and the total satisfaction rate was 94.0%.Conclusions Pacemaker system optimization can improve the heart function,activities of daily living and quality of life of elderly permanent pacemaker implant patients,and the patient satisfaction is higher.
