摘　　要：目的探讨自体骨移植在股骨干骨折不愈合中的疗效分析。方法回顾性分析2013-02至2018-02收治的13例股骨干骨折不愈合患者资料,其中男7例,女6例;左侧4例,右侧9例;年龄21-68岁,平均年龄46.8岁;伤时骨折按AO/OTA标准分型,32-A1型1例,32-A2型2例,32-A3型1例,32-B2型2例,32-B3型2例,32-C2型4例,32-C3型1例;骨折不愈合类型8例为萎缩型,5例为肥大型;13例患者中5例内固定断裂(其中3例为萎缩型,2例为肥大型),内固定断裂更换内固定加植骨,8例内固定未断裂中有3例为肥大型,更换内固定植骨,另外两例未更换处理后植骨,记录围手术期并发症及骨折愈合情况。结果12例患者一期愈合,1例患者进行再次手术后愈合,平均愈合时间10.3月。结论自体髂骨植骨内固定在治疗股骨干骨折不愈合中有良好的临床效果,且操作简单。Objective To analyze the efficacy of the autologous bone graft in femoral fracture nonunion.Methods We conducted a retrospective analysis on 13 patients with femoral fracture nonunion,who were treated between February 2013 and February 2018.7 of them were male,and 6 were female.4 of them suffered a fracture on the left side,and 9 of them suffered a fracture on the right side.They were aged 21 to 68,with a mean age of 46.8.According to the AO/OTA type definitions,1 patient had 32-A1,2 patients had 32-A2,1 patient had 32-A3,2 patients had 32-B2,2 patients had 32-B3,4 patients had 32-C2,and 1 patient had 32-C3.As for the fracture non-unions type,8 patients had the atrophic type,and 5 patients had the large fertilizer type.Among the 13 patients,5 had the internal fixation broken(3 of them had the atrophic type,and 2 of them had the large fertilizer type).The internal fixation was replaced for them,and bone grafts were added.Among the 8 patients that did not have the internal fixation intact,3 had the large fertilizer type,and internal fixation bone grafts were added for them.Another 2 received bone grafts without any replacement,and perioperative complications and fracture healing were observed.Results 12 patients experienced primary healing,1 patient healed after reoperation,and the average healing time was 10.3 months.Conclusion Autologous bone graft fixation in the treatment of femoral fracture nonunion delivers good clinical effects.
