摘 要:目的探讨外周血CD19^(+)CD24^(hi)CD38^(hi) B细胞亚群在煤工尘肺(CWP)壹期中表达水平的变化及临床意义。方法采用流式细胞技术检测150例CWP壹期患者(CWP组)、50名与CWP组相同接尘条件的健康井下接尘矿工(接尘组)及50名非接尘井上健康体检人员(对照组)外周血CD19^(+)CD24^(hi)CD38^(hi) B细胞亚群水平。结果与接尘组比较,CWP组患者外周血CD19^(+)CD24^(hi)CD38^(-)B细胞水平降低,CD19^(+)CD24^(int)CD38^(int)、CD19^(+)CD24^(hi)CD38^(hi) B细胞水平升高,差异均有统计学意义(P<0.05)。与对照组比较,CWP组患者外周血CD19^(+)CD24^(hi)CD38^(-)B细胞水平降低,CD19^(+)CD24^(hi)CD38^(hi) B细胞水平升高,差异均有统计学意义(P<0.05);接尘组外周血CD19^(+)CD24^(int)CD38^(int)、CD19^(+)CD24^(hi)CD38^(hi) B细胞水平降低,差异均有统计学意义(P<0.05)。CWP组岩尘亚组患者外周血CD19^(+)CD24^(int)CD38^(int)、CD19^(+)CD24^(hi)CD38^(hi) B细胞水平高于煤尘亚组,差异有统计学意义(P<0.05)。结论外周血CD19^(+)CD24^(hi)CD38^(hi) B细胞亚群水平变化可能与CWP患者肺部损伤程度有关,并参与CWP壹期发生发展过程。Objective To explore the level change of CD19^(+)CD24^(hi)CD38^(hi) B cell subsets in peripheral blood of the patients with stage Ⅰ of coal workers’pneumoconiosis(CWP),and its significance.Methods The levels of peripheral blood CD19^(+)CD24^(hi)CD38^(hi) B cell subsets were determined in 150 patients with stage Ⅰ of CWP(CWP group),50 healthy underground miners with similar dust exposure history(dust exposure group)and 50 ground workers without dust exposure history(control group)by flow cytometer.Results The results showed that the of CD19^(+)CD24^(hi)CD38^(-)B cells was reduced,and the CD19^(+)CD24^(int)CD38^(int),CD19^(+)CD24^(hi)CD38^(hi) B cells were significantly rised in CWP group compared with dust exposure group(P<0.05);while compared with control group,the level of CD19^(+)CD24^(hi)CD38^(-)B cells was also reduced and CD19^(+)CD24^(hi)CD38^(hi) B cells level in was rised as well in peripheral blood of CWP group.Meanwhile,the levels of peripheral blood CD19^(+)CD24^(int)CD38^(int) and CD19^(+)CD24^(hi)CD38^(hi) B cells were all lowed in dust exposure group(P<0.05),and its levels of peripheral blood CD19^(+)CD24^(int)CD38^(int) and CD19^(+)CD24^(hi)CD38^(hi) B cells were rised in silicon dusts exposed subgroup than those in coal dusts exposed subgroup(P<0.05).Conclusion The results suggested that the changes of CD19^(+)CD24^(hi)CD38^(hi) B cell subsets in peripheral blood may be related to the Ⅰ degree of lung injury in CWP patients,which may also be involved in the development process at the early stage of CWP.
