减张技术联合自体富血小板血浆在前交叉韧带重建术的应用及对膝屈曲角度的影响

摘　　要：目的探讨减张技术联合自体富血小板血浆(PRP)应用于前交叉韧带(ACL)重建术的效果及对膝屈曲角度的影响。方法回顾性分析2017年1月—2018年12月于笔者医院接受关节镜下减张技术重建前交叉韧带治疗(对照组,64例)及接受关节镜下减张技术联合PRP重建前交叉韧带治疗(研究组,58例)的ACL断裂患者的临床资料。对比2组患者术前、术后6个月及12个月膝关节功能(Lysholm评分),膝屈曲角度,步长(最大步长与最小步长)及步频以及MRI系统评分。结果术前、术后6个月及12个月,2组组内Lysholm评分、患膝屈曲角度比较差异显著(P<0.05),而步长、、步频以及MRI评分比较差异无统计学意义(P>0.05);2组组间比较术后6个月研究组Lysholm评分、患膝屈曲角度、MRI评分显著高于对照组(P<0.05),而2组步长、步频对比,差异无统计学意义;术后12个月2组Lysholm评分、患膝屈曲角度、步长、步频以及MRI评分比较,差异均无统计学意义(P>0.05)。结论对于前交叉韧带断裂患者,应用减张技术联合自体富血小板血浆重建前交叉韧带,可改善患者术后早期膝关节功能,促进韧带重塑。Objective To explore the effect of tension-relieving technique combined with autologous platelet-rich plasma(PRP)on anterior cruciate ligament(ACL)reconstruction and its effect on knee flexion angle.Methods Clinical data of patients with ACL rupture who received anterior cruciate ligament reconstruction under arthroscopy(control group,n=64)and anterior cruciate ligament reconstruction under arthroscopy combined with PRP(study group,n=58)in our hospital from January 2017 to December 2018 were retrospectively analyzed.The knee joint function(Lysholm score),knee flexion angle,step length(maximum step and minimum step)and stride frequency and MRI system score were compared between the two groups before surgery and at 6 months and 12 months after surgery.Results Before surgery and at 6 months and 12 months after surgery,there were significant differences in the Lysholm score,flexion angle of affected knee within the two groups(P<0.05),and there was no difference in step length,step frequency and MRI score(P>0.05).At 6 months after surgery,the Lysholm score,flexion angle of affected knee and MRI score in study group were higher than those in control group(P<0.05),but there were no significant differences in step length and step frequency between the two groups(P>0.05).At 12 months after surgery,there were no significant differences in the Lysholm score,flexion angle of affected knee,step length,step frequency and MRI score between the two groups(P>0.05).Conclusion For patients with anterior cruciate ligament rupture,tension-relieving technique combined with autologous platelet-rich plasma for anterior cruciate ligament reconstruction can improve knee joint function in the early postoperative period and promote the ligament remodeling.
