循经药熨结合弧度牵引治疗青年CSR的临床研究

摘　　要：目的探讨循经药熨结合颈椎弧度牵引治疗青年神经根型颈椎病(CSR)的远期疗效。方法选取2018年9月-2019年3月在天津中医药大学附属北辰区中医医院骨伤科诊断为CSR的青年患者60例,按随机数字表法分为治疗组和对照组各30例。对照组给予普通四头带牵引结合直线偏光治疗仪热敷,治疗组给予循经药熨结合颈椎弧度牵引。比较两组治疗2周后的VAS评分、颈椎功能障碍指数(NDI)、总有效率及治疗结束后6个月患者的复发率。结果治疗2周后,两组VAS评分、NDI和颈椎弧度值均明显优于治疗前,差异均有统计学意义(P<0.05)。治疗组VAS评分和NDI低于对照组,颈椎弧度值高于于对照组,差异均有统计学意义(P<0.05)。治疗组总有效率为93.33%,高于对照组的83.33%,差异有统计学意义(P<0.05)。随访6个月,治疗组复发率为3.33%,低于对照组的16.67%,差异有统计学意义(P<0.05)。结论循经药熨结合颈椎弧度牵引治疗青年CSR效果良好,远期复发率低,值得临床推广应用。Objective To investigate the long-term curative effect of medicnie ironing along meridians combined with cervical radian traction in the treatment of young cervical spondylotic radiculopathy(CSR).Methods Sixty young patients diagnosed with CSR in Department of Orthopedics and Traumatology,Beichen District Hospital of Traditional Chinese Medicine Affiliated to Tianjin University of Traditional Chinese Medicine from September 2018 to March 2019 were selected and divided into treatment group and control group with 30 cases in each group according to random number table method.The control group was given ordinary four headband traction combined with hot compress of linear polarizing therapeutic apparatus,the treatment group was given medicine ironing along meridians combined with cervical radian traction.The VAS score,cervical disability index(NDI) and total effective rate after 2 weeks of treatment and recurrence rate 6 months after the treatment of the two groups were compared.Results After 2 weeks of treatment,the VAS score and NDI and the value of cervical radian of the two groups were significantly better than those before treatment,and the differences were statistically significant(P<0.05),while the VAS score and NDI of the treatment group were lower than those of the control group,and the cervical radian value of the treatment group was higher than that of the control group,and the differences were statistically significant(P<0.05).The total effective rate of the treatment group was 93.33%,higher than 83.33% of the control group,and the difference was statistically significant(P<0.05).After 6 months of follow-up,the recurrence rate of the treatment group was 3.33%,which was lower than 16.67% of the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of young CSR by the medicine ironing along meridians combined with cervical radian traction had a good effect and low long-term recurrence rate,which was worthy of clinical promotion and application.
