线状银屑病一例及文献复习

摘　　要：目的了解线状银屑病的临床特点。方法回顾性分析成都市第二人民医院皮肤科经治的1例及既往国内文献报道的25例线状银屑病的临床资料。结果共收集26例线状银屑病,其中男15例,女11例,平均发病年龄(17.36±12.30)岁。皮损好发于四肢,躯干亦可受累。根据临床表现可分为单纯性线状银屑病,重叠性线状银屑病及线状银屑病并发其他沿Blaschko线分布的疾病3种类型。典型皮损组织病理改变示表皮角化过度伴角化不全,见Munro微脓肿,颗粒层变薄或缺失,棘层增厚,表皮突向下延长深入真皮,真皮乳头上方棘层变薄,毛细血管扩张,周围淋巴细胞浸润。根据患者年龄及皮损部位与严重程度选择不同的口服及外用药。结论线状银屑病好发于3个月~24岁人群,病程慢性,诊断主要依靠皮损表现和组织病理检查,治疗同一般银屑病。Objective To investigate the clinical characteristics of 1 inear psoriasis(LP).Methods One case of LP from Chengdu Second People’s Hospital and other 25 cases reported in the literature were retrospectively analyzed.Results A total of 26 cases of LP were collected,including 15 males and 11 females,and the average age of onset was(17.36±12.30)years.Skin lesions mainly occurred on limbs,while trunk might also be involved.According to the clinical manifestations,the disease was divided into three types:isolated LP,superimposed LP and LP with other skin diseases along the Blaschko line.Typical histopathological changes of the skin lesions showed hyperkeratosis with parakeratosis,Munro micro abscess,thinning or absence of granular layer,acanthosis,downward extension of rete ridges deeply into dermis,thinning of spinous layer above dermal papilla,telangiectasia and perivascular lymphocytic infiltration.Different oral and external drugs should be selected according to the patient’s age,lesion location and severity.Conclusion LP usually occurs in people aged from 3 months to 24 years,with a chronic course.The diagnosis of the disease mainly depends on the morphology of the skin lesions and histopathological examination,and the treatment is similar as that of classical psoriasis.
