中医推拿联合直肠滴药治疗小儿抗生素相关性腹泻的随机对照临床研究

摘　　要：目的观察中医推拿联合直肠滴药治疗小儿抗生素相关性腹泻(AAD)的临床疗效。方法采用随机数字表法将2016年8月至2018年8月该院收治的小儿AAD患儿80例分为观察组和对照组,每组40例。对照组采用常规西医疗法,观察组采用中医推拿联合直肠滴药治疗。观察2组患儿治疗后临床症状改善情况、肠道菌群及免疫功能相关指标等,并进行临床疗效评价。结果观察组患者治疗后大便频率和性状、腹痛和腹胀、食量评分均较治疗前及对照组治疗后明显降低,球/杆菌比例、分泌型免疫球蛋白A(SIgA)、二胺氧化酶、降钙素原均较治疗前及对照组治疗后明显改善,免疫球蛋白A(IgA)、IgG、IgM均较治疗前明显升高,且IgA、IgG、IgM均明显高于对照组,总有效率明显大于对照组,差异均有统计学意义(P<0.05)。结论采用中医推拿联合直肠滴药治疗小儿AAD能改善患儿临床症状及肠道菌群,调节体液免疫功能,临床疗效显著。Objective To observe the clinical efficacy of traditional Chinese medical massage combined with rectal dropping for treating antibiotic-associated diarrhea(AAD)in children.Methods Eighty children patients with AAD treated in this hospital from August 2016 to August 2018 were divided into the observation group and control group by adopting the random number table method,40 cases in each group.The control group adopted the conventional Western medical therapy.The observation group was treated with traditional Chinese medical message combined with rectal dropping.The improvement situation of clinical symptoms,intestinal flora related indicators and immune function related indicators after treatment were observed.Then the clinical effect was evaluated.Results After treatment,the scores of stool frequency and character,abdominal pain and abdominal distension and appetite in the observation group were significantly decreased compared with those before treatment in same group and after treatment in control group(P<0.05);the cocci/bacilli ratio,secreted immunoglobulin A(SIgA),diamine oxidase and PCT were significantly improved compared with those before treatment in same group and after treatment in control group(P<0.05);IgA,IgG and IgM in the observation group were significantly increased compared with those before treatment(P<0.05),moreover the IgA,IgG and IgM levels in the observation group were significantly higher than those in the control group,the total effective rate in the observation group was significantly greater than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Adopting the traditional Chinese medical massage combined with rectal dropping for treating AAD in children can improve the clinical symptoms and intestinal flora,regulate the humoral immune function,and the clinical curative effect is remarkable.
