摘 要:目的探讨超声雾化吸入与氧气雾化吸入在妇科全身麻醉(全麻)术后卧床患者中的临床应用效果。方法87例妇科全麻术后卧床患者,根据雾化治疗方法不同分为对照组(42例)和观察组(45例)。两组雾化药物相同,观察组患者应用氧气雾化方式,对照组采用超声雾化方式。比较两组患者治疗1周内排痰量以及治疗前后动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、血氧饱和度。结果两组患者雾化治疗后日排痰量逐渐降低,且观察组患者第2、3、4、5、6、7天以及排痰总量均高于对照组,差异均具有统计学意义(P<0.05)。治疗后,两组患者PaO2、血氧饱和度均高于治疗前,PaCO2低于治疗前,且观察组患者PaO2(96.14±2.72)mm Hg(1 mm Hg=0.133 kPa)、血氧饱和度(95.60±3.42)%均高于对照组的(88.32±2.60)mm Hg、(90.22±3.48)%,PaCO2(36.14±3.57)mm Hg低于对照组的(40.25±4.33)mm Hg,差异均具有统计学意义(P<0.05)。结论雾化治疗是妇科全麻术后卧床患者常用的治疗措施,氧气雾化方式对比超声雾化可显著提高患者术后排痰量,术后血气分析效果优于超声雾化方式,更加适用于该类患者治疗。Objective To discuss the clinical effect of ultrasonic nebulization and oxygen nebulization in bedridden patients after gynecological general anesthesia.Methods A total of 87 bedridden patients after gynecological general anesthesia were divided into control group(42 cases)and observation group(45 cases)according to different treatment methods.The nebulized drugs of the two groups were the same.The observation group was treated with oxygen nebulization,and the control group was treated with ultrasonic nebulization.The sputum output in 1 week of treatment,arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)and blood oxygen saturation before and after treatment were compared between the two groups.Results After atomization treatment,the daily sputum output of the two groups decreased gradually,and the sputum output and total sputum output of the observation group on the 2nd,3rd,4th,5th,6th and 7th day were higher than those of the control group,and the difference was statistically significant(P<0.05).After treatment,the PaO2 and blood oxygen saturation of the two groups were higher than those before treatment,and PaCO2 was lower than that before treatment.The PaO2(96.14±2.72)mm Hg(1 mm Hg=0.133 kPa)and blood oxygen saturation(95.60±3.42)%of the observation group were higher than(88.32±2.60)mm Hg and(90.22±3.48)%of the control group,and PaCO2(36.14±3.57)mm Hg was lower than(40.25±4.33)mm Hg of the control group.All the difference was statistically significant(P<0.05).Conclusion Nebulization therapy is a commonly used method for bedridden patients after gynecological general anesthesia.Compared with ultrasonic nebulization,oxygen nebulization can significantly increase the patient’s postoperative sputum production,and its postoperative blood gas analysis is better than ultrasonic nebulization,which is more suitable for the treatment of such patients.
