疏风解毒胶囊对慢性阻塞性肺疾病急性加重期sTREM-1、YKL-40、SP-D、Fib的影响

摘　　要：目的:观察疏风解毒胶囊对风热犯肺证慢性阻塞性肺疾病急性加重期(AECOPD)患者血清可溶性髓样细胞触发受体-1(sTREM-1)、人软骨糖蛋白-39(YKL-40)、肺泡表面活性蛋白(SP-D)、纤维蛋白原(Fib)的影响,探讨其作用机制。方法:将60例风热犯肺证AECOPD患者随机分为对照组与治疗组,每组30例,对照组采用西医基础治疗,而治疗组则在西医常规治疗的基础上加服疏风解毒胶囊,观察疗程10d。观察两组治疗前后的中医证候积分变化,血清sTREM-1、YKL-40、SP-D、Fib水平的变化,观察其疗效。结果:治疗后,两组中医证候积分均较治疗前显著改善(P<0.01),其中咳嗽、发热、口渴、汗出4项积分治疗组的改善优于对照组(P<0.05),两组治疗后sTREM-1、YKL-40、SP-D、Fib均较治疗前显著降低(P<0.01),且治疗组优于对照组(P<0.05)。结论:疏风解毒胶囊联合西药治疗能迅速改善风热犯肺证AECOPD患者的临床症状,降低血清炎症因子sTREM-1、YKL-40、SP-D、Fib的水平,减轻气道炎症反应。Objective:To observe the effects of Shufeng Jiedu Capsule on serum sTREM-1,YKL-40,SP-D,Fib of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients with wind-heat invading lung syndrome,and explore its mechanism of action.Methods:A total of 60 cases of AECOPD patients with wind-heat invading lung syndrome were randomly divided into treatment group and control group with 30 cases in each group,the control group was treated with basic western medicine while the treatment group was treated with Shufeng Jiedu Capsule on the basis of western medicine for ten days.The changes in TCM syndrome scores and serum levels of sTREM-1,YKL-40,SP-D,Fib before and after treatment in the two groups were observed to evaluate the efficacy.Results:After treatment,the TCM syndrome scores of the two groups were significantly improved(P<0.01),and the improvement of cough,fever,thirst and sweating in TCM syndrome scores was better in the treatment group than that in the control group(P<0.05).The levels of STREM-1,YKL-40,SP-D and Fib in the treatment group were significantly reduced(P<0.01),which were superior than the control group(P<0.05).Conclusion:Shufeng Jiedu Capsule combined with western medicine can rapidly improve the clinical symptoms of AECOPD patients with wind-heat invading lung syndrome,reduce the levels of serum inflammatory factors sTREM-1,YKL-40,SP-D and Fib,and reduce airway inflammation reaction.
