加味温胆汤治疗儿童单纯性肥胖症的疗效及对胰岛素抵抗、脂代谢的影响

摘　　要：目的探讨加味温胆汤治疗单纯性肥胖症患儿的疗效及对胰岛素抵抗、脂代谢的影响。方法2018年5月—2019年5月收治的单纯性肥胖症患儿120例,经随机数字表法分为对照组、观察组各60例。对照组患儿接受单纯性肥胖症常规治疗,观察组患儿在对照组基础上加入加味温胆汤进行联合干预。持续治疗14 d后,对比2组患者的胰岛素抵抗程度、脂质代谢水平、整体疗效差异。结果治疗后,观察组患儿空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)的水平低于对照组患儿(P<0.001);观察组患儿治疗后的三酰甘油(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平低于对照组患儿,高密度脂蛋白胆固醇(HDL-C)的水平高于对照组患儿(P<0.05);观察组疗效分级、总有效率优于对照组患儿(P<0.05)。结论加味温胆汤可进一步优化单纯性肥胖症患儿的病情,具体可能与其减轻胰岛素抵抗及脂质代谢紊乱的作用相关。Objective To investigate the effect of Wendan decoction on in the treatment of simple obesity children and its influence on insulin resistance and lipid metabolism.Methods 120 cases of obese children admitted to our hospital from May 2018 to May 2019 were divided into control group and observation group,with 60 cases in each,by random number table.Children in control group received routine treatment,and those in observation group were treated with Wendan decoction on the basis of control group.14 days after continuous treatment,the differences in insulin resistance,lipid metabolism and overall efficacy between two groups were compared.Results After treatment,levels of FINS and HOMA-IR in observation group were lower than those in control group,levels of TG,TC,LDL-C in observation group were lower than those in control group,HDL-C level in observation group was higher than that in control group,and grade of efficacy and total effective rate in observation group were better than those of control group(P<0.05).Conclusion Wendan decoction can further optimize the condition of children with simple obesity,which may be related to the effect of reducing insulin resistance and lipid metabolism disorder.
