腹腔镜下肾盂翻瓣输尿管吻合术治疗二次肾盂输尿管连接部狭窄效果分析

摘　　要：目的总结腹腔镜下肾盂翻瓣输尿管吻合术治疗二次肾盂输尿管连接部狭窄的安全性及有效性。方法回顾性分析2018-02—2019-10间在扶沟县第二人民医院和郑州大学第一附属医院因二次肾盂输尿管连接部狭窄行腹腔镜下肾盂翻瓣输尿管吻合术治疗的15例患者的临床资料。狭窄段长度3~7 cm(中位数4 cm)。患者均为中重度肾积水,且既往行肾盂输尿管连接部成形术。结果手术时间65~130 min,平均85.5 min。随访时间为6~24个月,平均10.00个月。11例(73.33%)患者术后肾积水和临床症状消失,随访期间肾积水未再复发。4例(26.67%)患者术后转变为轻度肾积水,且临床症状消失,随访期间肾积水未再加重。结论对于需二次肾盂输尿管连接部成形手术的患者,行腹腔镜下肾盂翻瓣输尿管吻合术,创伤小、术后恢复快,效果显著。Objective The clinical data of 15 patients treated by laparoscopic ureterostomy of the second people's Hospital of Fugou County and the first affiliated Hospital of Zhengzhou University were retrospectively analyzed.Narrow segment length 3~7 cm(median 4 cm).The patients were all moderate and severe hydronephrosis and underwent ureteropelvic junction.Methods Retrospectively analyzed the clinical data of 15 patients who underwent laparoscopic ureteral anastomosis under the laparoscopic ureteropelvic urethra from February 2018 to October 2019 in our hospital.The length of the narrow segment is 3~7 cm(median 4 cm).All patients had moderate to severe hydrocephalus and previous ureteropelvic ureteroplasty was performed.Results The operation time was 65~130 min,the average was 85.5 min the follow-up time was 6~24 months and the average was 10.00 months.Postoperative hydronephrosis and clinical symptoms disappeared in 11 patients(73.33%),and no recurrence occurred during follow-up.Four patients(26.67%)were treated with mild hydronephrosis and the clinical symptoms disappeared.The hydronephrosis was not aggravated during follow-up.Conclusion Laparoscopic ureteropelvic anastomosis with small trauma and quick postoperative recovery was performed in patients who needed secondary ureteropelvic junction molding.
