摘 要:目的探讨不同维持量枸橼酸咖啡因治疗胎龄<34周早产儿呼吸暂停的效果与安全性。方法选取2019年2月~2020年2月鹰潭市人民医院收治的80例胎龄<34周呼吸暂停早产儿作为研究对象,采用随机数字表法分为对照组和观察组,每组各40例。两组患者均以枸橼酸咖啡因为治疗药物,对照组使用剂量为5 mg/(kg·d),观察组剂量为10 mg/(kg·d),比较两组治疗总有效率、治疗时间、不同阶段呼吸暂停次数和出现不良反应的概率。结果观察组的治疗总有效高于对照组,差异有统计学意义(P<0.05);观察组的机械通气时间、低流量吸氧时间、住院时间短于对照组,差异有统计学意义(P<0.05);观察组24、48、72 h后呼吸暂停次数少于对照组,且用药48 h、用药72 h后呼吸暂停发作次数少于本组用药24 h后,用药72 h后呼吸暂停发作次数少于本组用药48 h后,差异有统计学意义(P<0.05);观察组的不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论10 mg/(kg·d)的枸橼酸咖啡因维持剂量,对胎龄<34周的呼吸暂停早产儿的治疗效果更佳,发生不良反应的情况更少。Objective To explore the effect and safety of different maintenance doses of Caffeine Citrate in the treatment of premature infants with gestational age<34 weeks with apnea.Methods A total of 80 premature infants with apnea less than 34 weeks of gestational age admitted to Yingtan People′Hospital from February 2019 to February 2020 were selected as the research objects and divided into the control group and the observation group according to the random number table method,with 40 patients in each group.Both groups were treated with Caffeine Citrate as a therapeutic agent.The dosage of the control group was 5 mg/(kg·d),and the dose of the observation group was 10 mg/(kg·d).The total effective rate,treatment time,number of apnea at different stages and the probability of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The mechanical ventilation time,low-flow oxygen ventilation time and hospital stay time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).The number of apnea after 24,48,72 hours in the observation group was fewer than that in the control group,and the differences were statistically significant(P<0.05).The total incidence of adverse reactions of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The maintenance dose of 10 mg/(kg·d)of Caffeine Citrate is more effective in the treatment of premature infants with apnea<34 weeks of gestational age and less adverse reactions.
