根管术、牙周及调[牙合]联合治疗逆行性牙髓炎的效果

摘　　要：目的探究根管术、牙周及调[牙合]联合治疗逆行性牙髓炎的效果。方法选取2018年4月15日~2019年6月18日于抚顺中医院收治的112例逆行性牙髓炎患者为研究对象。按随机数字表法分为研究组与对照组,每组各56例。研究组采用根管术、牙周及调[牙合]联合治疗,对照组采用根管治疗术。比较两组肿瘤坏死因子-α(TNF-α)水平、白细胞介素-2(IL-2)水平、牙齿松动度评分、叩痛度评分及不良反应总发生率。结果两组治疗后TNF-α、IL-2水平低于治疗前,差异有统计学意义(P<0.05);研究组治疗后TNF-α、IL-2水平低于对照组,差异有统计学意义(P<0.05);两组治疗后牙齿松动度、叩痛度评分低于治疗前,差异有统计学意义(P<0.05);研究组治疗后牙齿松动度、叩痛度评分低于对照组,差异有统计学意义(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论根管术、牙周及调牙合联合治疗逆行性牙髓炎患者,可有效减轻患者牙齿的松动程度,缓解疼痛,提升疗效。Objective To investigate combined effect of root canal surgery,periodontal and occlusal adjustment in treatment of retrograde pulpitis.Methods A total of 112 patients with retrograde pulpitis who were admitted to Fushun Hospital of Chinese Medicine from April 15,2018 to June 18,2019 were selected as study subjects.They were divideded into study group and control group according to random number table method,with 56 cases in each group.The study group adopted the combined treatment of root canal therapy,periodontal therapy and occlusal adjustment,while the control group adopted root canal therapy.The levels of tumor necrosis factor-α(TNF-α),interleukin-2(IL-2),tooth mobility score,percussion pain score and the total incidence of adverse reactions were compared between the two groups.Results The levels of TNF-αand IL-2 in the two groups after treatment were lower than those before treatment,and the differences were statistically significant(P<0.05).The levels of TNF-αand IL-2 in the study group were lower than those in the control group after treatment,and the differences were statistically significant(P<0.05).After treatment,the scores of tooth mobility score and percussion pain in both groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the scores of tooth mobility score and percussion pain in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Comparison of the total incidence of adverse reactions between the two groups,the difference was not statistically significant(P>0.05).Conclusion Application of combination of root canal surgery,periodontal and occlusal adjustment in treatment of patients with retrograde pulpitis can effectively reduce the degree of tooth loosening,relieve pain and improve the curative effect.
