颅脑损伤术后脑梗死患者D-二聚体及C-反应蛋白检测价值分析

摘　　要：目的探讨D-二聚体和C-反应蛋白在颅脑损伤(TBI)术后脑梗死患者中的检测价值。方法选择贵州航天医院2017年6月—2020年6月收治的68例TBI术后脑梗死患者为研究对象,根据相关标准对患者病灶体积和严重程度进行分级,测定不同分级患者D-二聚体、C-反应蛋白水平。结果不同梗死体积患者D-二聚体、C-反应蛋白水平差异显著,同时随着梗死体积的增加,D-二聚体、C-反应蛋白水平相应增加;不同严重程度患者D-二聚体、C-反应蛋白水平差异显著,同时随着病情的加重,D-二聚体、C-反应蛋白水平相应增加,差异均有统计学意义(P<0.05)。D-二聚体、C-反应蛋白水平与颅脑损伤术后脑梗死患者病灶体积大小、NIHSS评分呈显著正相关(P<0.05)。结论对于颅脑损伤术后脑梗死患者测定发病24小时内C-反应蛋白、D-二聚体水平,对病情及预后评估均有一定的指导价值。Objective To investigate the value of D-dimer and C-reactive protein in patients with cerebral infarction after craniocerebral injury(TBI).Methods 68 patients with cerebral infarction after TBI in our hospital from June 2017 to June 2020 were selected as the research objects.The lesion volume and severity were graded according to the relevant standards,and the levels of D-dimer and C-reactive protein in patients with different grades were measured.Results the levels of D-dimer and C-reactive protein in patients with different infarct volumes were significantly different.With the increase of infarct volume,the levels of D-dimer and C-reactive protein increased.The levels of D-dimer and C-reactive protein in patients with different severity were significantly different.At the same time,with the aggravation of the disease,the levels of D-dimer and C-reactive protein increased,according to Spearman correlation analysis The levels of D-dimer and C-reactive protein were positively correlated with the size of the lesion and NIHSS score,and the differences were statistically significant(P<0.05).Conclusion For patients with cerebral infarction after craniocerebral injury operation,the determination of C-reactive protein and D-dimer levels within 24 hours after the onset of the disease has certain guiding value for the evaluation of the patient's condition and prognosis.
