玻璃体腔注射低剂量曲安奈德治疗白内障术后葡萄膜炎临床评价

摘　　要：目的探讨玻璃体腔注射低剂量(1 mg)曲安奈德治疗白内障术后葡萄膜炎的临床疗效,以及对炎性因子水平的影响。方法回顾性分析北京中医医院顺义医院2017年12月至2018年12月收治的白内障术后葡萄膜炎患者94例(94眼),按治疗方案的不同分为高剂量组(48例,48眼)与低剂量组(46例,46眼),高剂量组患者给予玻璃体腔注射曲安奈德注射液4 mg,低剂量组患者给予玻璃体腔注射曲安奈德注射液1 mg。两组均持续治疗1个月。结果治疗后,低剂量组视力恢复(>0.5)率明显高于高剂量组(P<0.05),眼压、房水蛋白浓度、白细胞介素6、肿瘤坏死因子-α、γ干扰素水平均明显低于高剂量组(P<0.05);两组治疗前后角膜内皮计数相当(P>0.05);低剂量组不良反应发生率明显低于高剂量组(P<0.05)。结论玻璃体腔注射曲安奈德1 mg治疗白内障术后葡萄膜炎,可有效恢复视力,减轻炎性反应,且对眼压无影响。Objective To investigate the clinical efficacy of intravitreal injection of low-dose(1 mg)triamcinolone acetonide(TA)in the treatment of patients with uveitis after cataract surgery and its effect on the levels of inflammatory factors.Methods Totally 94 patients(94 eyes)with uveitis after cataract surgery admitted to Shunyi Hospital of Beijing Traditional Chinese Medicine Hospital from December 2017 to December 2018 were retrospectively analyzed.According to different treatment schemes,they were divided into the high-dose group(48 cases,48 eyes)and the low-dose group(46 cases,46 eyes).The patients in the high-dose group were intravitreally injected with 4 mg of Triamcinolone Acetonide Injection,while the patients in the low-dose group were intravitreal injected with 1 mg of Triamcinolone Acetonide Injection.Both groups were continuously treated for one month.Results After treatment,the visual acuity recovery(>0.5)rate of the low-dose group was significantly higher than that of the high-dose group(P<0.05),the intraocular pressure,aqueous protein concentration,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and interferon-γ(IFN-γ)in the low-dose group were significantly lower than those in the high-dose group(P<0.05).There was no statistically significant difference in the corneal endothelial cell count between the two groups before and after treatment(P>0.05).The incidence of adverse reactions of the low-dose group was significantly lower than that of the high-dose group(P<0.05).Conclusion Intravitreal injection of 1 mg of triamcinolone acetonide in the treatment of patients with uveitis after cataract surgery can effectively recover the visual acuity without raising intraocular pressure.
