高流量鼻导管吸氧联合精细化护理在低氧性呼吸衰竭患者中的应用

摘　　要：目的探究对低氧性呼吸衰竭的患者采用高流量鼻导管吸氧联合精细化护理的效果。方法选择2018年1月至2020年1月期间本院收治的64例低氧性呼吸衰竭患者为研究对象,采用数字随机法将所有患者分为观察组和对照组,每组32例。对照组患者采用常规的鼻导管吸氧或面罩吸氧,护理方面采用对症处理、心理护理、健康宣教等常规护理措施;观察组患者采用高流量鼻导管吸氧和精细化护理。记录两组患者高流量鼻导管吸氧和精细化护理后48 h的相关指标、治疗后的患者舒适度、护理后生活质量评定。结果在经过高流量鼻导管吸氧和精细化护理后,观察组患者各项指标均显著优于对照组(均P<0.05);高流量鼻导管吸氧治疗的患者无不耐受现象发生,观察组患者舒适率为87.5%(28/32),对照组为62.5%(20/32),两组比较差异有统计学意义(χ^(2)=5.33,P=0.02);护理后,观察组患者各项生活质量评分均较对照组显著提高(均P<0.05)。结论低氧性呼吸衰竭的患者采用高流量鼻导管吸氧和精细化护理,可明显改善患者的临床症状,提高患者生活质量,同时又可以避免并发症的发生。Objective To explore the effect of high flow nasal catheter flow combined with meticulous nursing in patients with hypoxic respiratory failure.Methods Sixty-four patients with hypoxic respiratory failure admitted to our hospital from January 2018 to January 2020 were selected as the research objects.All patients were divided into observation group and control group by the random number table method,32 cases in each group.The patients in the control group were given routine oxygen inhalation by nasal catheter or mask,and routine nursing measures such as symptomatic treatment,psychological nursing,and health education;the patients in the observation group were given high flow nasal catheter for oxygen inhalation and meticulous nursing.The relevant indexes 48 h later,comfort degree of patients after treatment,and quality of life after nursing were recorded in the two groups.Results After high flow nasal catheter flow and meticulous nursing,all indexes of patients in the observation group were significantly better than those in the control group(all P<0.05);patients treated by high flow nasal catheter oxygen inhalation had no intolerance,the comfort rate of the observation group was 87.5%(28/32),and that of the control group was 62.5%(20/32)(χ^(2)=5.33,P=0.02).After nursing,the quality of life of patients in the observation group was significantly higher than that in the control group(P<0.05).Conclusion High flow nasal catheter flow combined with meticulous nursing can improve the clinical symptoms of patients with hypoxic respiratory failure,improve their quality of life,and avoid complications.
