接纳与承诺疗法联合造口袋更换口诀卡在肠造口患者中应用价值分析

摘　　要：目的探究肠造口患者应用接纳与承诺疗法联合造口袋更换口诀卡的价值。方法纳入2019年3月至2020年5月本院行应永久造口患者70作为研究对象,将患者分为两组,各35例。对照组实施常规护理及健康教育,研究组实施接纳与承诺疗法联合造口袋更换口诀卡;对比两组患者各项心理适应能力、对造口袋更换知识和技能的掌握情况及并发症处理情况。结果研究组正性情绪、负性情绪、社会生活适应、总评分分别为(21.06±3.94)分、(18.67±3.64)分、(25.49±4.62)分、(65.22±12.20)分,均高于对照组(16.73±2.56)分、(12.34±2.28)分、(19.34±3.87)分、(48.41±8.71)分,差异均有统计学意义(均P<0.05);研究组造口袋揭取、造口底盘裁剪、造口周围皮肤护理、造口袋更换顺序、并发症处理分别为(12.35±5.34)分、(13.87±6.57)分、(14.16±4.67)分、(16.34±8.75)分、(18.45±5.35)分,均高于对照组(4.12±3.12)分、(7.35±4.24)分、(5.67±3.64)分、(8.14±5.16)分、(9.65±4.65)分,差异均有统计学意义(均P<0.05)。结论在肠造口患者中应用接纳与承诺疗法联合造口袋更换口诀卡缓解其负性情绪,提高其对社会适应能力,同时提高患者对造口袋更换的知识认识,提高生活质量。Objective To explore the value of applying accepting and promising therapy combined with pocket changing formula card in patients taking enterostomy.Methods From March 2019 to May,2020,70 patients with permanent stoma treated at our hospital were included as the study objects.The patients were divided into a control group and a study group,with 35 cases in each group.The control group was given routine nursing and health education,and the study group was given accepting and promising therapy and pocket changing formula cards.The psychological adaptation abilities,knowledge about changing stoma pockets,command of skills,and complication treatment were compared between the two groups.Results The scores of positive emotion,negative emotion,and social life adaptation and the total score were higher in the study group than in the control group[(21.06±3.94)vs.(16.73±2.56),(18.67±3.64)vs.(12.34±2.28),(25.49±4.62)vs.(19.34±3.87),and(65.22±12.20)vs.(48.41±8.71);all P<0.05].The scores of bag opening,stoma chassis cutting,skin care around stoma,bag replacement sequence,and complication treatment were higher in the study group than in the control group[(12.35±5.34)vs.(4.12±3.12),(13.87±6.57)vs.(7.35±4.24),(14.16±4.67)vs.(5.67±3.64),(16.34±8.75)vs.(8.14±5.16),and(18.45±5.35)vs.(9.65±4.65);all P<0.05].Conclusion Accepting and promising therapy combined with pocket changing formula card for patients taking enterostomy can alleviate their negative emotions and improve their social adaptability,knowledge of pocket changing,and quality of life.
