脑卒中患者急救期主要照顾者心理应激现状及影响因素分析

摘　　要：目的调查脑卒中患者急救期主要照顾者心理应激现状,并分析其影响因素。方法以本院2019年4月至2020年4月期间接收的220例脑卒中急性发作患者和主要照顾者为调查对象,收集患者及主要照顾者一般资料,运用亲属应激量表(RSS)调查患者主要照顾者心理应激现状,运用单因素和多因素logistic回归分析影响患者主要照顾者心理应激水平的相关因素。结果脑卒中患者急救期主要照顾者应激量表评分为(14.53±3.25)分;单因素分析,脑卒中患者急救期主要照顾者心理应激得分与患者已就诊时间、居住地、主要照顾者性别、年龄、婚姻状况、与患者关系、是否全职照顾、是否需要照顾其他人、经济负担、卒中知识了解程度、认为患者是否有生命危险、社会支持度等因素具有明显相关性(均P<0.05);多因素logistic回归分析,患者已就诊时间、主要照顾者性别、年龄、与患者关系、是否全职照顾、是否需要照顾其他人、经济负担、卒中知识了解程度、认为患者是否有生命危险、社会支持度等因素均为影响脑卒中患者急救期主要照顾者心理应激水平的独立性因素。结论脑卒中患者急救期主要照顾者心理应激处于中等偏低水平,与患者发病情况、照顾者性别、年龄、照顾工作、经济负担、疾病认知和社会支持度等因素密切相关,临床中应重视对照顾者的心理干预,减轻照顾者心理应激。Objective To investigate the psychological stress status of the main caregivers of stroke patients during the emergency period and analyze its influencing factors.Methods Two hundred and two acute stroke patients admitted to our hospital from April,2019 to April,2020 and their main caregivers were selected as the investigation objects.The patients'and main caregivers'general data were collected.Relative Stress Scale(RSS)was used to investigate the caregivers'psychological stress.Single factor and multi-factor logistic regressions were used to analyze the related factors that affected the caregivers'psychological stress.Results The caregivers'RSS score was(14.53±3.25).Single factor analysis showed that the caregivers'RSS score was related to the patients'treatment time,residence,and their gender,their age,their marital status,their relationship with the patients,full-time care or not,whether they needed to take care of other people,their financial burden,their stroke knowledge,whether the patients were considered life-threatening,social support,and other factors(all P<0.05).Multivariate logistic regression analysis showed that the patients'treatment time,the caregivers'gender,age,and relationship with the patients,full-time care or not,whether they needed to take care of other people,financial burden,stroke knowledge,whether the patients were considered life-threatening,social support,and other factors were independent factors that affected their psychological stress.Conclusion The psychological stress of the main caregivers of stroke patients is at a moderate to low level,and is closely related to the patients'morbidity,their gender,their age,care work,economic burden,disease knowledge,and social support.The caregivers should be psychologically intervened to reduce their stress.
