通窍鼻炎颗粒冲洗鼻腔对鼻-鼻窦炎伴腺样体肥大患儿术后康复的疗效及机制研究

摘　　要：目的:观察通窍鼻炎颗粒冲洗鼻腔对鼻-鼻窦炎伴腺样体肥大患儿术后康复的疗效及作用机制。方法:将90例腺样体切除术后患儿随机分为对照组和观察组,各45例。对照组患儿给予糠酸莫米松鼻喷雾剂,观察组患儿给予通窍鼻炎颗粒冲洗鼻腔,疗程均为2周。观察治疗前后两组患儿的鼻腔鼻窦结局测试-20(SNOT-20)、鼻内镜检查评分量表(Lund-Kennedy)评分,血清及鼻分泌液中肿瘤坏死因子α(TNF-α)、白细胞介素-17(IL-17)的含量。评价两组治疗的安全性。结果:观察组总有效率97.8%(44/45),高于对照组的82.2%(37/45)(P<0.05)。治疗后,观察组患儿SNOT-20和Lund-Kennedy评分、血清及鼻分泌液中TNF-α和IL-17含量低于对照组患儿(P<0.05)。对照组患儿出现5例不良反应,观察组未见明显不良反应。结论:通窍鼻炎颗粒冲洗鼻腔可有效提高鼻-鼻窦炎伴腺样体肥大患儿术后的康复疗效,其作用机制可能与改善血清及鼻分泌液中TNF-α、IL-17含量有关。Objective:To observe the therapeutic effect and mechanism of tongqiao biyan granules irrigateing nasal cavity for postoperative rehabilitation of children with nasosinusitis and adenoid hypertrophy.Methods:90 cases were randomly divided into the control group and the observation group,45 cases in each group.Mometasone furoate nasal spray was given in the control group,and tongqiao biyan granules irrigating the nasal cavity was given in the observation group,both for 2 weeks.The sino-nasal outcome test-20(SNOT-20)score,nasal endoscopy Lund-Kennedy score,TNF-αand IL-17 levels in serum and nasal secretions were observed before and after treatment.The safety of the treatment of both groups were evaluated.Results:The total effective rate of the observation group was 97.8%(44/45),which was higher than that of the control group(82.2%,37/45)(P<0.05).After treatment,the SNOT-20 scores and Lund-Kennedy scores,TNF-αand IL-17 levels in serum and nasal secretions of the patients in the observation group were lower than those of the patients in the control group(P<0.05).5 cases of adverse reactions were reported in the control group,and no obvious adverse reactions were reported in the observation group.Conclusion:Tongqiao biyan granules irrigating nasal cavity can effectively improve the postoperative rehabilitation effect of children with nasosinusitis and adenoid hypertrophy,and its mechanism may be related to the improvement of TNF-αand IL-17 level in serum and nasal secretions.
