大芬戈尔德菌致肱骨骨髓炎1例报道并文献复习

摘　　要：目的了解大芬戈尔德菌的特点及其引起疾病的临床特征,提高对该菌引起感染性疾病的认识。方法回顾性分析1例诊断为大芬戈尔德菌骨髓炎患者的临床资料,结合文献复习探讨大芬戈尔德菌感染性疾病的特征。结果该例患者临床表现和实验室检查符合大芬戈尔德菌感染性骨髓炎的特征,脓液和组织培养有大芬戈尔德菌生长,术后无复发。复习、汇总文献及该例患者,共32例(男性13例,女性10例,9例未提及),其中骨和关节(包括假体)感染16例、感染性心内膜炎5例、蜂窝织炎3例、乳腺感染3例、其他5例;临床表现主要与受累组织系统相关。31例经外科手术联合抗菌药物治疗后痊愈,1例血流感染患者死亡。结论骨关节感染,尤其假体植入后关节感染,应警惕大芬戈尔德菌所致,必要时给予经验性抗厌氧菌治疗。Objective To examine the characteristics of Finegoldia magna infection for improving the awareness of this disease.Methods One case of humerus osteomyelitis caused by F.magna was reported in a male patient.Other infectious diseases caused by F.magna were also searched from literature and reviewed.Results The diagnosis was confirmed as F.magna humerus osteomyelitis based on clinical manifestations and laboratory tests,including positive culture of F.magna from pus and tissue.No recurrence was observed at follow-up.A total of 32 cases(13 males,10 females,9 unknown)of F.magna infection were reviewed(including this one).The specific site of infection included infective endocarditis(n=5),bone and joint including prosthesis(n=16),subcutaneous mass(n=3),breast(n=3),and others(n=5).Clinical symptoms varied with the organs involved.Majority of the patients(n=31)were cured by surgery combined with antimicrobial therapy.Only one patient died with bloodstream infection.Conclusions Clinicians should be aware of the possibility of F.magna infection in case of bone and joint infection,especially following prosthesis implantation.Empirical anti-anaerobic therapy can be prescribed if necessary.
