益肾降浊汤在慢性肾脏病3、4期肾性贫血治疗中的应用效果

摘　　要：目的观察益肾降浊汤治疗慢性肾脏病(chronic kidney disease,CKD)3、4期肾性贫血(renal anemia,RA)的效果。方法选择CKD3、4期RA患者80例,依据随机数字表法分为联合组及常规组,每组40例,两组患者均进行常规治疗,联合组在常规治疗的基础上给予益肾降浊汤治疗。分别于治疗前、后测定血肌酐(serum creatinine,Scr)、尿素氮(blood urea nitrogen,BUN)、超敏C反应蛋白(high sensitivity C-reactive protein,hs-CRP)、红细胞(red blood cell,RBC)、血红蛋白(hemoglobin,Hb)、血细胞比容(hematocrit,HCT)、铁蛋白(serum ferritin,SF)、总铁结合力(total iron binding capacity,TIBC)、铁调素(hepcidin)水平;并对治疗前后两组患者中医证候积分值变化进行评定。结果治疗后两组患者Scr、BUN及hs-CRP水平较治疗前下降,RBC、Hb、HCT水平较治疗前上升(P均<0.05);联合组治疗后Scr、BUN及hs-CRP水平均低于常规组,RBC、Hb、HCT水平高于常规组(P均<0.05);治疗后两患者组SF水平较治疗前上升,TIBC及hepcidin水平较治疗前下降(P均<0.05);联合组治疗后SF水平高于常规组,hepcidin水平低于常规组(P均<0.05);治疗后两组患者中医证候积分值较治疗前下降,治疗后联合组中医证候积分值低于常规组(P均<0.05)。结论CKD3、4期RA患者在常规治疗过程中联合益肾降浊汤进行治疗,能有效改善其贫血状态,下调患者铁调素水平,治疗效果优于常规治疗,值得临床推广应用。Objective To observe the therapeutic effect of Yishen Jiangzhuo decoction in the treatment of renal anemia(RA)in patients with stage 3,4 chronic kidney disease(CKD).Methods A total of 80 RA patients with stage 3,4 CKD who were admitted in our hospital,were included in the study and randomized into the combination group and the routine group.The patients in the two groups were given routine treatments.On the above basis,the patients in the combination group were given Yishen Jiangzhuo decoction.The fully automatic biochemical analyzer was used to detect Scr,BUN,hs-CRP,RBC,Hb,HCT,SF,TIBC and hepcidin.TCM syndrome scores before and after treatment in the two groups were evaluated.Results Scr,BUN and hs-CRP levels after treatment in two groups were significantly reduced when compared with before treatmen(tP all<0.05).Scr,BUN and hs-CRP levels after treatment in the combination group were significantly lower than those in the routine group(P all<0.05).But those levels after treatment in two groups were significantly elevated when compared with before treatment(P all<0.05).RBC,Hb and HCT levels after treatment in the combination group were significantly higher than those in the routine group(P all<0.05).However,SF level after treatment in two groups was significantly elevated when compared with before treatment,while TIBC and hepcidin levels were significantly reduced(P all<0.05).SF level after treatment in the combination group was significantly higher than that in the routine group,but hepcidin level was significantly lower than that in the routine group(P all<0.05).TCM syndrome scores after treatment in two groups were significantly reduced when compared with before treatment.TCM syndrome scores after treatment in the combination group were significantly lower than those in the routine group(P all<0.05).Conclusion On the basis of routine treatments,Yishen Jiangzhuo decoction in the treatment of RA patients stage 3,4 CKD can effectively improve the anemia state,and downregulate the hepcidin level,with obvious treatme
