摘　　要：目的探讨血栓弹力图检测在指导心脏外科术后患儿血制品使用的价值。方法选取247例心脏外科手术患儿,分别检测术后输血前后血栓弹力图检测并统计所输注的血浆和血小板的数量,比较组间的差异。血液制品使用前R值正常组168例,血浆输注前R值延长组79例,K值正常组患儿105例,K值延长组患儿142例,MA正常组患儿121例。MA值降低组126例患儿。结果 R值延长组输注血浆后R值明显降低(P<0.05),R值延长组血浆输注量明显高于R值正常组(P<0.05);K值延长组输注血浆后K值明显降低(P<0.05),K值延长组血浆输注量明显高于K值正常组(P<0.05);MA值降低组输注PLT后MA明显升高(P<0.05),MA值降低组PLT输注量明显高于MA值正常组(P<0.05),MA值降低和MA值正常组患儿血浆输注量无明显差异(P>0.05)。结论血栓弹力图检测中的R值、K值、MA值对心脏外科手术患儿术后血浆和PLT的输注具有指导意义。Objective To analyze the value of thromboelastography(TEG)in guiding the blood product transfusion in children after cardiac surgery.Methods In 247 cardiac surgery children,we analyzed their TEG after their operations and statisticsed the quantity of transfused blood plasma and platelelet.Patients(n=247)were divided into normal R group(n=168)and extended R group(n=79);normal K group(n=105)and extended K group(n=142);normal MA group(n=121)and decreased MA group(n=126).Results The value of R was decreased in the extended R group after plasma transfusion(P<0.05).The value of K was decreased in the extended K group after plasma transfusion(P<0.05).The plasma transfusion volumes in the extended R group and the extended K group were more than those in the normal R group and the normal R group obviously(P<0.05).The value of MA was increased in the decreased MA group after the platelelet transfusion(P<0.05).The PLT transfusion volume in the decreased MA group was more than those in the normal MA group(P<0.05).There was no difference of the plasma transfusion volume between the decreased MA group and the normal MA group(P>0.05).Conclusion R,K and MA in TEG can guide the blood product transfusion in children after cardiac surgery.
