用冷生理盐水冲洗法对接受单极高频电刀凝切手术的会厌囊肿患者进行创面冷疗的效果观察

摘　　要：目的:观察用冷生理盐水冲洗法对接受支撑喉镜下单极高频电刀凝切手术的会厌囊肿患者进行创面冷疗的效果。方法:选择2015年1月至2020年3月灌南县人民医院收治的62例会厌囊肿患者作为研究对象。将其随机分为盐水冷疗组(31例)和对照组(31例)。对两组患者均进行支撑喉镜下单极高频电刀凝切手术。在进行囊肿电凝切除及创面电凝止血时,对对照组患者不进行特殊处理,应用冷生理盐水反复对盐水冷疗组患者的术区进行冲洗。在完成止血处理后,使用冷生理盐水灌浸其咽喉腔。然后比较两组患者术后疼痛、创面肿胀的程度、创面白膜脱落的时间和创面愈合的时间。结果:在术后1 d、3 d、5 d,盐水冷疗组患者的VAS评分均低于对照组患者,P<0.05。盐水冷疗组患者术后创面白膜脱落的时间和创面愈合的时间均短于对照组患者,其中存在术后1~3 d创面明显肿胀情况患者的占比低于对照组患者,P<0.05。结论:用冷生理盐水冲洗法对接受支撑喉镜下单极高频电刀凝切手术的会厌囊肿患者进行创面冷疗能明显减轻其术后疼痛及创面肿胀的程度,促进其创面的愈合。Objective:To observe the effect of cold saline irrigation on wound cold therapy of epiglottic cyst patients who underwent monopole high-frequency electrosurgical coagulation under supported laryngoscope.METHODS:From January 2015 to March 2020,62 cases of patients with cyst were selected as the research subjects.They were randomly divided into saline water cooling group(n=31)and control group(n=31).Both groups were treated with single-pole high-frequency electrosurgical coagulation under propping laryngoscope.When cyst electrocoagulation resection and wound electrocoagulation hemostasis were performed,patients in the control group were not treated with special treatment,and the operative area of patients in the salt-water cooling group was repeatedly flushed with cold normal saline.After hemostasis,the laryngopharyngeal cavity was infused with cold saline.Then,the postoperative pain,degree of wound swelling,time of leukoplema on the wound and time of wound healing were compared between the two groups.RESULTS:At 1,3 and 5 d after operation,VAS scores in saline-water cooling group were lower than those in control group(P<0.05).The time of postoperative white membrane exfoliation and wound healing in the salt-water cooling group was shorter than that in the control group,and the proportion of patients with obvious wound swelling 1~3 days after surgery was lower than that in the control group(P<0.05).Conclusion:Cold saline irrigating for wound cold therapy of epiglottic cyst patients who underwent monopole high-frequency electrosurgical coagulation under support laryngoscope can significantly reduce the degree of postoperative pain and wound swelling,and promote wound healing.
