摘 要:目的评估国产Columbus三维电生理标测系统指导阵发性室上性心动过速(PSVT)射频消融的安全性和有效性。方法回顾分析2016年1月~2019年12月于郑州大学附属洛阳中心医院心内科确诊为PSVT并接受Columbus三维标测系统指导下RFCA术的患者210例,统计患者的手术时间、X线曝光时间、即刻消融成功率和并发症发生率。结果其中房室结折返性心动过速(AVNRT)158例,左侧旁路47例,右侧旁路5例。除1例患者消融失败外,其余患者均获成功。AVNRT的平均手术时间(68.7±13.5)min,X线曝光时间为(3.8±2.6)min,左侧旁路平均手术时间(78.7±11.2)min,X线曝光时间为(10.8±5.7)min,右侧旁路平均手术时间为(90.1±22.6)min,X线曝光时间为(3.85±2.2)min。无术中并发症发生。选择2016年1月前的接受传统X线消融的100例AVNRT患者与国产三维组AVNRT患者对比,国产三维消融组整体手术时间长于二维消融组(P<0.05),但X线曝光时间三维组较二维组明显缩短(P<0.01)。结论国产Columbus三维电生理标测系统指导PSVT消融具有良好的安全性及有效性。Objective To evaluate the safety and effectiveness of three-dimensional electrophysiological mapping system of Columbus in guiding radiofrequency ablation of paroxysmal supraventricular tachycardia(PSVT).Methods From January 2016 to December 2019,210 patients who were diagnosed with PSVT and underwent RFCA under the guidance of Columbus three-dimensional mapping system in the Department of Cardiology,Luoyang Central Hospital Affiliated to Zhengzhou University were retrospectively analyzed,and the operation time,X-ray exposure time,success rate of immediate ablation and complication rate were counted.Results There were 158 cases of atrioventricular nodal reentrant tachycardia(AVNRT),47 cases of left-sided bypass and 5 cases of right-sided bypass.The ablation was successful in all but one patient.The average operation time of AVNRT was 68.7±13.5 min,X-ray exposure time was(3.8±2.6)min,average operation time of left bypass was(78.7±11.2)min,X-ray exposure time was(10.8±5.7)min,average operation time of right bypass was(90.1±22.6)min,and X-ray exposure time was(3.85±2.2)min.No intraoperative complications occurred.One hundred AVNRT patients who underwent conventional X-ray ablation before January 2016 were selected to compare with AVNRT patients in the domestic three-dimensional ablation group.The overall operation time of the domestic three-dimensional ablation group was longer than that of the two-dimensional ablation group(P<0.05),but the X-ray exposure time of the three-dimensional group was significantly shorter than that of the two-dimensional group(P<0.01).Conclusion The homemade Columbus three-dimensional electrophysiological mapping system has good safety and effectiveness in guiding PSVT ablation.To evaluate the safety and efficiency of Columbus three-dimensional electroanatomic mapping system in guiding radiofrequency ablation of paroxysmal supraventricular tachycardia(PSVT).Methods A retrospective analysis of 210 patients who were diagnosed with PSVT and received RFCA guilded by Columbus three-dimen
