扩张型心肌病患者血清半乳糖凝集素-3、分形趋化因子及可溶性sST2的变化及临床意义

摘　　要：目的探讨扩张型心肌病(DCM)患者血清半乳糖凝集素-3(Gal-3)、分形趋化因子(FKN)及可溶性sST2(sST2)的变化及临床意义。方法选取2018年1月至2019年12月于山东第一医科大学附属济南人民医院收治的110例DCM患者作为DCM组,并根据美国纽约心脏协会(NYHA)心功能分级分为Ⅱ级组、Ⅲ级组和Ⅳ级组。选取同期行体检的110例心功能正常者作为对照组。检测两组患者血清Gal-3、FKN和sST2水平,研究Gal-3、FKN和sST2与DCM间的关系。结果DCM组患者Gal-3、FKN和sST2水平均显著高于对照组(P<0.05)。DCM患者血清Gal-3、FKN和sST2水平随心功能分级的增加而升高(P<0.05)。Spearman相关性分析,血清Gal-3、FKN和sST2水平与DCM患者NYHA分级呈现出正相关性(P<0.05)。Gal-3对扩张型心肌病的曲线下面积(AUC)为0.978,95%CI(0.964,0.993),此时Gal-3截断值为11.20 ng/ml,敏感度、特异性为0.920、0.900。FKN对扩张型心肌病的AUC为0.923,95%CI(0.891,0.955),此时FKN截断值为0.915μg/L,敏感度、特异性为0.700、0.955。sST2对扩张型心肌病的AUC为0.860,95%CI(0.811,0.908),此时sST2截断值为13.975 ng/ml,敏感度、特异性为0.745、0.773。结论DCM患者机体内血清Gal-3、FKN和sST2存在高表达,且与患者心功能受损具有密切相关性,其有望成为DCM疾病诊治和评估预后的重要指标。Objective To discuss the changes and clinical significance of serum galectin-3(Gal-3),fractalkine(FKN)and soluble suppression of tumorigenicity 2(sST2)in patients with dilated cardiomyopathy(DCM).Methods DCM patients(n=110)were chosen into DCM group from Jinan People’s Hospital affiliated to Shandong First Medical University from Jan.2018 to Dec.2019,and further divided,according to NYHA classification,into NYHA gradeⅡgroup,NYHA gradeⅢgroup and NYHA gradeⅣgroup.At the same period,110 cases with normal heart function were chosen into control group.The levels of Gal-3,FKN and sST2 were detected,and relationship among Gal-3,FKN,sST2 and DCM were studied in all groups.Results The levels of Gal-3,FKN and sST2 were significantly higher in DCM group than those in control group(P<0.05).The levels of Gal-3,FKN and sST2 increased as the increases of NYHA classification(P<0.05).The results of Spearman correlation analysis showed that the levels of Gal-3,FKN and sST2 and NYHA classification had a positive correlation in DCM patients(P<0.05).The area under curve(AUC)of Gal-3 for DCM was 0.978(95%CI:0.964~0.993),cutoff value was 11.20 ng/ml,sensitivity was 0.920 and specificity was 0.900.AUC of FKN was 0.923(95%CI:0.891~0.955),cutoff value was 0.915μg/L,sensitivity was 0.700 and specificity was 0.955.AUC of sST2 was 0.860(95%CI:0.811~0.908),cutoff value was 13.975 ng/ml,sensitivity was 0.745 and specificity was 0.773.Conclusion The expressions of Gal-3,FKN and sST2 were higher in DCM patients and are closely correlated to heart function damage,which are possibly important indexes for diagnosis,treatment and prognosis reviewing of DCM.
