师瑞华理筋法运用下肢力线选穴联合温针灸对退行性膝骨关节病前内侧痛的疗效观察

摘　　要：目的:探讨师瑞华理筋法运用下肢力线选穴联合温针灸对退行性膝骨关节病(KOA)前内侧痛的疗效。方法:随机数字表法将2017年7月~2019年10月收治的116例KOA前内侧痛患者分为2组,各58例。对照组给予下肢力线选穴温针灸治疗,观察组在对照组基础上联合下肢力线施师瑞华理筋法,比较两组患者中医证候积分、VRS量表、Lequesne指数、生存质量量表简表评分(QOL—BREF)的差异。结果:两组中医证候各项积分低于治疗前(P<0.05),观察组各项积分低于对照组(P<0.05)。治疗后两组患者疼痛等级评分、关节活动度评分、Lequesne日常活动受限指数较治疗前降低;且观察组明显低于对照组(P<0.05);两组治疗后生存质量(QOL—BREF)评分高于治疗前(P<0.05),观察组各项评分均显著高于对照组(P<0.05)。结论:下肢力线选穴采用师瑞华理筋法联合温针灸治疗可显著改善KOA引起的前内侧痛、活动受限等临床症状,改善日常生活质量,值得临床推广。Objective:To explore the effect of SHI Ruihua’s method on degenerative knee osteoarthritis(KOA)pain by using lower extremity acupoint selection combined with warming acupuncture.Methods:116 patients with anterior medial pain of KOA admitted from July 2015 to October 2019 were divided into 2 groups with 58 patients in each group by random number table method.The controlled group was given acupoint selection and warming acupuncture treatment for lower limb strength line,and the observation group was combined with ruihua tendon reconditioning method on the basis of the controlled group to compare the differences between the two groups in TCM syndrome score,VRS scale,Lequesne index and QOL-BREF.Results:The scores of TCM syndromes in the two groups were lower than those before treatment(P<0.05),and those in the observation group were lower than those in the controlled group(P<0.05).After treatment,pain grade score,joint range of motion score and Lequesne limited daily activity index in both groups were lower than before treatment.The observation group was significantly lower than the controlled group(P<0.05).Quality of life(QOL-BREF)scores of the two groups after treatment were higher than those before treatment(P<0.05),and all scores of the observation group were significantly higher than those of the controlled group(P<0.05).Conclusion:SHI Ruihua Lijin Method combined with warming acupuncture and moxibustion treatment for acupoint selection of lower extremity force line can significantly improve the clinical symptoms such as anteromedial pain and restricted activity caused by KOA,and improve the quality of daily life.
