摘 要:目的探讨子野数对胸段食管癌静态调强放疗计划剂量分布的影响,为临床治疗提供参考。方法选取胸段食管癌患者20例,保持射野角度及优化参数不变,子野数分别设为10、20、30、40、50,为每例患者设计5个静态调强计划,并对5个计划的靶区最大剂量、最小剂量、平均剂量、适形指数(CI)和均匀指数(HI),以及危及器官受照射剂量和体积、机器跳数进行比较。结果30子野计划的剂量学参数优于10子野计划和20子野计划,有统计学意义(均P<0.05),与40子野、50子野计划相比较,差异无统计学意义(P>0.05)。脊髓受量D_(max)随着子野数增加而减小(P<0.05),机器跳数随着子野数的增加而增加(P<0.05)。结论30子野数胸段食管癌静态调强计划能够满足临床剂量学要求,拥有较好的靶区适形度及均匀度,能有效保护危及器官,同时可以减少治疗时间,可以作为胸段食管癌静态调强放疗计划的设计参数。Objective To investigate the influence of segment numbers on the planning of static intensity-modulated radiation therapy for thoracic esophageal cancer,to provide references for clinical treatment.Methods 20 patients with thoracic esophageal cancer were elected.The segment numbers were set to 10、20、30、40、50,and 5 plans were designed for every patient with beam angles and optimization target parameters of plan constant.The target maximum dose,minimum dose,mean dose,conformity index(CI)and homogeneity index(HI),irradiated volume and dose of organ at risk,and monitor units among the plans were compared.Results The dosimetric parameters of 30 segments plan were better than those of 10 segments plan and 20 segments plan(P<0.05);there was no significant difference between 30 segments plan with 40 segments plan and with 50 segments plan(P>0.05).The max dose of spinal cord decreased with the increase of segment numbers(P<0.05);the number of machine units increased with the increase of segments number(P<0.05).Conclusion 30 segments IMRT plan for thoracic esophageal cancer have good uniformity and conformity of dose distribution,can meet the requirements of clinical dosimetry,protect effectively the organs at risk and reduce the treatment time.It can be used as the design parameters of static IMRT plan for thoracic esophageal cancer.
