摘 要:症状性椎动脉颅内段非急性闭塞(SNAIVAO)患者尽管规律服用药物治疗仍可反复发生颅内缺血事件,且侧支代偿不足者预后较差。SNAIVAO患者血管内开通治疗的优势明显,但该手术在治疗过程中仍存在不足之处。光学相干断层成像(OCT)技术在血管内治疗颅内闭塞动脉中可提供较高的辅助作用,并可降低术中并发症。笔者回顾性报道1例应用OCT技术辅助血管内开通治疗SNAIVAO的治疗过程及预后,并通过复习相关文献进一步分析了SNAIVAO患者应用OCT辅助血管内开通术的诊治体会。Patients with symptomatic non-acute intracranial vertebral artery occlusion(SNAIVAO)can still have recurrent intracranial ischemia events despite regular medication therapy,and the prognosis is poor with insufficient collateral compensation.Endovascular recanalization has obvious advantages in SNAIVAO patients,but there are still shortcomings during the treatment process.Optical coherence tomography(OCT)can be used as an adjunct in the treatment of intracranial artery occlusion and reduce intraoperative complications.The author retrospectively reported the treatment process of OCT assisted endovascular recanalization and prognosis in 1 case of SNAIVAO.Diagnosis and treatment experience of SNAIVAO with OCT assisted endovascular recanalization were further analyzed by reviewing relevant studies.
