摘　　要：目的探讨长链非编码RNA CASC7(LncRNA CASC7)与微小RNA-27b-3p(miR-27b-3p)在肺结核外周血单核细胞中的表达及意义。方法结核菌H37Rv、H37Ra分别感染单核细胞,采用qRT-PCR法检测CASC7、miR-27b-3p的表达量;双荧光素酶报告基因实验检测CASC7与miR-27b-3p的靶向关系;选取2018年3月至2019年10月本院收治的肺结核患者120例为研究组,根据活动性肺结核诊断标准将肺结核患者分为非活动性肺结核组60例、活动性肺结核组60例,同时选取同期健康体检者60例为对照组;采用实时荧光定量聚合酶链反应(qRT-PCR)法检测CASC7、miR-27b-3p的表达量;ROC分析CASC7与miR-27b-3p对肺结核的诊断价值。结果与感染前比较,H37Rv、H37Ra感染后单核细胞中CASC7的表达水平显著降低(P<0.05),miR-27b-3p的表达水平显著升高(P<0.05);双荧光素酶报告基因实验证实CASC7可靶向结合miR-27b-3p;与对照组比较,研究组患者外周血单核细胞中CASC7的表达水平显著降低(P<0.05),且活动性肺结核组显著低于非活动性肺结核组(P<0.05),miR-27b-3p的表达水平显著升高(P<0.05),且活动性肺结核组显著高于非活动性肺结核组(P<0.05);ROC分析显示CASC7的敏感度为57.50%,特异度为91.67%,AUC面积为0.781;miR-27b-3p的敏感度为57.50%,特异度为81.67%,AUC面积为0.665;联合检测的敏感度为96.67%,特异度为60.00%,AUC面积为0.802。结论肺结核外周血单核细胞与结核菌感染单核细胞中CASC7表达下调,miR-27b-3p表达上调,对肺结核辅助诊断及靶向治疗均具有一定参考价值。Objective To explore the expression and significance of LncRNA CASC7 and miR-27b-3p in peripheral blood mononuclear cells of pulmonary tuberculosis.Methods Mycobacterium tuberculosis H37Rv and H37Ra respectively infected monocytes,and qRT-PCR method was used to detect the expression of CASC7 and miR-27b-3p.The dual luciferase reporter gene experiment detected the targeting relationship between CASC7 and miR-27b-3p.A total of 120 patients with pulmonary tuberculosis admitted to our hospital from March 2018 to October 2019 were selected as the research group.According to the diagnostic criteria of active pulmonary tuberculosis,patients with pulmonary tuberculosis were divided into 60 patients in the inactive pulmonary tuberculosis group and 60 in the active pulmonary tuberculosis group.At the same time,60 healthy people were selected as the control group.The expression of CASC7 and miR-27b-3p were detected by qRT-PCR method.It used ROC to analyze the diagnostic value of CASC7 and miR-27b-3p for pulmonary tuberculosis.Results Compared with before infection,the expression level of CASC7 in monocytes after H37Rv and H37Ra infection was significantly reduced(P<0.05),and the expression level of miR-27b-3p was significantly increased(P<0.05).Dual luciferase reporter gene experiments confirmed that CASC7 could target and bind to miR-27b-3p.Compared with the control group,the expression level of CASC7 in the peripheral blood mononuclear cells of the study group was significantly reduced(P<0.05),and the active pulmonary tuberculosis group was significantly lower than that of the inactive pulmonary tuberculosis group(P<0.05),the expression level of miR-27b-3p was significantly increased(P<0.05),and the active pulmonary tuberculosis group was significantly higher than the inactive pulmonary tuberculosis group(P<0.05).ROC analysis showed that CASC7 had a sensitivity of 57.50%,a specificity of 91.67%,and an AUC area of 0.781.The sensitivity of miR-27b-3p was 57.50%,the specificity was 81.67%,and the AUC area was 0.665.The sensi
